| Objective: To study the pathogenesis of polymyositis(PM) by using immunohistochemistry staining and electron microscope technique. To prove the therapeutical effect of warming the kidney, invigorating primordial energy, strengthening the spleen and promoting tissue regeneration(herbs of Wen Shen Bu Yuan Jian Pi Sheng Ji, WSBYJPSJ) to cure PM from several facets such as biochemistry, immunity and pathology. and to try to explore the action mechanism of the therapy in order to offer the target of looking for the ideal and reliable prescription to cure PM.Methods: Polymyositis guinea pigs were induced by coxsachie virus B1, taking advantage of rabbits' homogenate and complete Freunds adjuvant. Took a part of muscles from the ideal model of PM to observe by high power microscope after histology and enzyme histochemistry staining. After fixation of part muscle sample, used monoclonal antibody fluorescent immunohistochemistry staining of anti-γand anti-λimmune globulin light chain to research it. Made ultra-lamina before electron staining in order to observe and photo it by electron microscope. Forty polymyositis guinea pigs were divided at random into A(n=15), B(n=15), C(n=10) groups. A group was treated with herbs of warming the kidney, invigorating primordial energy, strengthening the spleen and promoting tissue regeneration 50ml per kg everyday, B group with tripterygium wilfordii 50mg per kg everyday. As a blank control group, C group received no treatment. To compare the symptom, muscle zymogram and pathologic change among three groups after four weeks' treatment.Result: the muscle transverse section showed several muscle fiber bundles whose boundaries are clear. The small vessel wall in muscles showed local pachynsis and acidophilic degeneration, infiltrated by inflammatory cells. There were a lot of eosinophilia bodies around the epimysium. The pressed metamorphotic plasmocyte nuclei were seen aside some bodies. Fresh necrotic muscle fibers , acidophilic degenerated muscle fibers and basophilic muscle fibers having hypertrophic nuclei were seen at the same time. With Congo red staining, Rusell bodies showed positive reaction and the pachynsis parts of small vessel wall were stained red like cloud and mist. There were also positive depositions of Congo red staining in perimysium near the small vessels. Russell bodies were symbols of immune globulin that were secreted abnormally by plasma cells, which hinted that the depositions in small vessel walls and perimysium stemmed from immune globulin secreted by plasma cells. Fluorescent immune histochemistry staining showed anti-λmonoclonal antibody staining was positive. There were ringed and irregular fruorescent substance around the small vessel and outside the muscle fibers. There were granules showed strong positive reaction in epimysium. In these parts, anti-γimmune Fluorescent staining showed light postive reaction. Electron microscope showed lots of depositions like strip and ball lied mainly around normal small vessels, in or on the muscle fibers. Destroied myofilament and atrophic muscle fibers formed finger processes. After treatment with herbs of warming the kidney, invigorating primordial energy, strengthening the spleen and promoting tissue regeneration, the amyosthenic symptom of A group was more alleviated than that of B and C groups. The mobility and food-intake of A group were better than B and C groups. After treatment for four weeks, the muscle zymogram of A and B groups were more decreased than that of C group, the difference was significant (p<0.01, comparison of AST between B and C, p<0.05). Compared to CPK, LDH, AST before treatment, the three kinds of enzymes were all decreased after treatment in group A and B, The difference was significant (p<0.05). The muscle zymogram was lower in A group than that in B group after treatment, and the difference was significant (p<0.05). But the muscle zymogram showed an aggravating trend in C group.
