Objectives: To examine the alteration of the cytokines in different hyperplastic phases of breast intraductal epithelial cell carcinomatous change and their relationship to dendritic cell (DC) and clinical data, and provide academic basis for clinical immunotheraphy.Methods: The cytokines VEGF, IL-10, TNF-alpha, and GM-CSF were examined by immunohistochemistry in 150 routinely paraffin embedded specimens including 15 cases of normal breast (normal group), 20 cases of intraductal hyperplasia (IDH group), 29 cases of atypical intraductal hyperplasia (AIDH group), 9 cases of intraductal carcinoma and 6 cases of early infiltrating ductal carcinoma (DCIS group), and 71 cases of infiltrating ductal carcinoma (IDC group). The positive rates and labeling index of VEGF, IL-10, TNF-alpha, and GM-CSF were investigated and compared with DC and clinico-pathological parameters. Results:1. Positive rates of the cytokines in different hyperplastic phases of breast intraductal epithelial cell carcinomatous changeThe positive rates of VEGF in normal, IDH, AIDH, DCIS, and IDC group were 0,10.0%,20.7%,33.3%,and,70.4% respectively.IL-10 were 13.3%,25.0%, 27.6%,73.3% and 69.0% respectively.TNF-alpha were 0, 0, 7.1%, 66.7% and 88.7% respectively.GM-CSF were 13.3%, 20.0%, 29.6%, 60.0% and 76.1% respectively. The rates of four cytokines were significantly different among five groups. The positive rates of VEGF and GM-CSF in IDC group were significantly higher than those of the other four groups (P<0.05 for all), but no difference among the other 4 groups. The rates of IL-10 in DCIS and IDC groups weresignificantly higher than those of the other three groups (P<0.05 for all), but no difference among the other 3 groups. TNF-alpha was negative in both normal and IDH groups, but the significant differences among the other three groups were found (P0.01 for all).2. Labeling indexes of the cytokines in different hyperplastic phases of breast intraductal epithelial cell carcinomatous changeThe labeling indexes of VEGF in normal, IDH, AIDH, DCIS, and IDC group were 0.00?.00,0,20?.62,0.66?.37,1.25?.49 and 3.56?.29 respectively. IL-10 were 0.27?.70,0.60?.14,0.96?.68,2.00?.96 and 2.75?.73 respectively. GM-CSF were 0.20?.56, 0.45?.05, 0.93?.06, 2.27?.81 and 3.54?.16 respectively.TNF-alpha were 0, 0, 0.11?.416, 1.80?.01,and 3.65?.87 respectively.The labeling indexes of the four cytokines were significantlydifferent among five groups (P<0.01 for all) .The labeling indexes of VEGF inIDC group is significantly higher than other four ones (P<0.05 for all) .Both IL-10and GM-CSF in DCIS and IDC were significant higher than other three groups(P<0.05 for all) . There were siganifant differences in labeling indexes ofTNF-alpha among AIDH, DCIS, and IDC groups (P<0.05 for all) .3. Correlation among the four cytokines in different hyperplastic phases of breast intraductal epithelial cell carcinomatous changeThere were significantly positive correlations in the labeling indexes of the cytokines among the 5 groups besides no correlation between VEGF and GM-CSF (P<0.05 for all). The labeling index of VEGF in IDH group wassignificantly positive correlated with that of IL-10 (rs=0.535,P<0.05) .In DCIS group, IL-10 was significantly negative correlated with TNF-alpha (rs=-0.537, P<0.05), and significantly positive relation between TNF-alpha and GM-CSF was found (rs=0.894,P<0.01) . In IDC group, there were good positive correlatoins inVEGF with IL-10 (rs=0.328,P=0.005) ,and significantly negative correlation in VEGF with TNF-alpha and GM-CSF respectively (rs=-0.257, -0.362, P-0.030, 0.002, respectively) ,and positive correlation in TNF-alpha with GM-CSF(rs=0.488,P=0.000) .4.
