Objective To discuss the probability and advantage of dislocating the tibiotalar Joint laterally during the operations with clinical observation on the tri-malleolar fractures in 60 cases. The relative problems about ankle fractures are also discussed.Method The 60 cases are divided into 2 groups at random:the model and the normal. In order to gain adequate exposure and anatomic reduction for the posterior tibial margin ,the tibiotalar joint are dislocated laterally after the bi-malleolus are exposed in the model group. In the normal group,the posterior tibial margin are exposed by turning down the distal fragment of the fibular.All the 60 cases are treated with plaster cast after operations for 6 weeks and washed by the Chinese herbs called HuoxueZhitongSanResult All the cases are evaluated with the AOFAS score system and the diffrence between the two groups is not statistically significant(p>0.05).Conclusion Dislocating the tibiotalar jiont laterally during the operative treatment for the trimalleolar fractures is a good method .It can afford adequate exposure and is easy to practice.
