Objective: In this paper, we examined the variation of plasminogen activator inhabitor-1 (PAI-1 for short) and endothelin (ET for short) and analyzed the relationship between serum uric acid level and PAI-1 , ET, glucose and insuline metabolism, lipid metabolism disorder in middle-aged and old -aged male patients with hyperuricemia. The aim is to explore the possible damage effects on the inside of blood vessels caused by hyperuricemia.Method: Choosing 138 male patients over 35 years old with hyperuricemia in Qingdao area under study. Dividing this patients into four groups, 42 patients with sole hyperuricemia as Group UA-1 , 32 patients with hyperuricemia and impaired glucose tolerance as Group UA-2, 34 patients with hyperuricemia and hyperlipidaemia as Group UA-3 and 30 patients with hyperuricemia , hyper lipidaemia and impaired glucose tolerance as Group UA-4. We choose 40 people coming from the medical examination in our hospital as the reference group. Those people have no coronary heart disease, hypertension, diabetes, and their blood lipids and serum uric acid level are normal. The general conditions as age, body mass index, waist hip ratio, systolic blood pressure, diastolic blood pressure are examined. PAI-1, ET level and other biochemical variables including serum uric acid level, total cholesterol, triglyceride, high density lipoprotein-cholesterol, low density lipoprotein-cholesterol, fasting plasma glucose, postprandial 2h plasma glucose, fasting blood insulin, blood urea nitrogin and creatinine are determined.Result: Based on our research, the levels of PAI-1 and ET in group UA-1 are notably higher than the reference group (p<0. 01) under no significant difference in the circumstance such as age, systolic blood pressure,diastolic blood pressure, TC, TG, HDL, LDL, FBG, 2hBG, BUN, Cr . The levels of PAI-1 and ET in group UA-2, UA-3, UA-4 are notably higher than those of the reference group (p<0. 01) . PAI-1 level in group UA-4 is notably higher than group UA-1(p<0. 05). ET level in group UA-4 is notably higher than group UA-1 (p<0. 01). After controlling BMI>WHR, DBF, UA is positively relevant to the vitality of PAI-1 (r=0. 345, p<0. 01) , UA is positively relevant to the vitality of ET (r=0. 369, p<0. 01). Further multivariate regression analysis revealed that TG, LDL, Bun , Cr , PAI-1 and ET levels contributed to the UA levels (r2=0.43 ,p<0.001).Conclusion: Hyperuricemia is one of the independent risk factors of vascular endothelial cells functional disorder. Vascular endothelial cells may be more damaged, especially companied with hyperlipidaemia and hyperglucose level.
