ObjectiveThe rate of the incidence of the pancreatic cancer was increasing. The pancreatic cancer had become one of the important death reasons in the digestive tract cancer. But the early diagnosis of pancreatic cancer was difficult. Only a few patients could receive radical resection. But the tumor could recur and metastasis after years, the long effect was dissatisfied. About 70% of these patients could not receive tumor resection. They did not receive effective palliate therapy. The objective of this study is to sum up and compare the outcome of different modalities for pancreatic cancer patients and explore a better manner mainly on operation to cure the pancreatic cancer.MethodsClinical data of 545 pancreatic cancer cases treated in our department between January 1990 and March 2003 were analyzed with SPSS 10.0 statistical software package retrospectively. The survival rates of various kinds of treatment methods were compared by Kaplan-Meier method,ResultsOf 545 patients, there were 346 male and 199 female. The cancer located in the head of the pancreas was 388 patients, 129 were located in the body and tail of the pancreas, and 28 were allover the pancreas. Of 385 patients who received operation, 130 underwent radical resection, 34 palliative resection, 183 palliative by-pass, 38 exploration operation, only 26 systemic chemotherapy, 134 no special treatment. The rate of resection was 42.6%, no operation death, 11 patients were died in in-hospital time. 502 patients had follow-up result, the time of follow-up was 4 days to 83 months and the rate of follow-up was 92.11%. The survival rate of the radical resection group had significant different statistics compared with the other 5 groups. The survival rate of palliate resection and chemotherapy after operation also had significant different statistics compared with the other palliate treatment groups. And the other groups had no significant different statistics compared with each other. The l-,3-,and 5-y survival rates of the radical resection group were 61.19%, 21.79%,and 10.63 % Respectively.ConclusionsEarly diagnosis of pancreatic is essential to improve the effect of treatment. Radical resection can improve long-term survival of pancreatic cancer patient. Extended lymphadenectomy can improve the survival rate to early pancreatic cancer but do not increase the complications and mortality, but it was not commended to the other patients. Radical resection is important to the pancreatic cancer which had attack the blood vessel, if the radical resection can not achieve, resection the vessel is not need. The palliate resection and chemotherapy after operation had some effect. The other palliative treatments have very limited effect but can improve the quality of life of the patients. The effective of chemotherapy to the pancreatic cancer can be validated further.
