The metastasis of tumor is a procedure controlled by multiple genetic factors. Epithelial-cadherin ( E-CD) is considered as an important neoplasitc metastasis inhibitory oncogene which can improve cellular adherence and maintain the normal shape of the tissue. The downward expression and loss of E-CD can both lead to diffusion of tumor cell. Type IV collagen (Col IV ) is the basic fracture bone and main component of the basemen membrane (BM) and in the normal condition BM is a stable and compact continuous membrane which can prevent the transport of the large molecular and cell. Tumor can destroy its continuous character then invasive and metastasis.PurposeTo study the relationship of neoplasitc metastasis inhibitory oncogene E-CD and the expression of type IV collagen, the main component of the basemen membrane, in the larynx carcinoma with the jugular lymph node metastasis.Materials and methodThe materials include the resected sample of 51 super- glottic larynx carcinoma from CMU in 1998-2001 and 4 para carcinoma normal tissues as compared tissue sample. We use the immuno-histological chemical method ( SABC method, two-step method) to ditect the expression of E-CD and ColIV- Then using test to analysis its result.ResultIn the 51 super- glottic larynx carcinoma, the positive expression of E-CD is 30 The E-CD positive expression rate in phase I and H of larynx carcinoma is ( ++)54.55%(6/11),( +)36.36%; in phase land IV is(.++)17.50% (7/40),(+) 32. 50% (13/40). The difference is significant ( p <0. 05). In the group with no metastasis of larynx carcinoma in the jugular lymph node, the E-CD positive expression rate is ( ++ )45.45% (10/22) , ( +)45.45% (10/ 22) ; In the group with the metastasis of larynx carcinoma in the jugular lymph node, the E-CD positive expression rate is ( ++ ) 10. 34% (3/29) , ( + )24. 14% (7/29) , the difference is significant (p <0.01).The expression of ColIV is 31. In phase I and n of larynx carcinoma, the expression rate of ColIV is ( ++ )72. 73% (8/11),( + )27. 27% (3/11); in phase land IV expression rate of ColIV is CoUV ( ++ ) 27. 50% (11/40) , ( + )22.50% (9/40) ; the difference is significant( p <0. 05 ) . Among the larynx carcinoma group with no metastasis in the jugular lymph node, the expression rate of ColIV is ColIV( ++ ) % 54. 55% (12/22) , ( + ) 36. 36% (8/12) ; among the larynx carcinoma group with the metastasis in the jugular lymph node, the complete expression rate of ColIV is ( ++ )24. 13(7/29) , ( + ) 13. 79% (4/29) ; the difference is significant ( p <0.01).E-CD(-)expression and the ColIV(-) expression, in the group of larynx carcinoma with no metastasis of the jugular lymph node is4.55% (1/22) , in the group with the metastasis of the jugular lymph node is 41.14% (12/29) ; E-CD (++) expression and the ColIV ( ++ ) expression, in the larynx carcinoma group with no metastasis of the jugular lymph node is 36. 36% ( 8/22) , in the group with the metastasis of the jugular lymph node is 6.90% (2/29) ,the combined expression of E-CD and Col IV in metastasis of the jugular lymph node and no metastasis of the jugular lymph node, the difference is significant ( p < 0. 01).DiscussionA mount of test suggests, E-CD has relationship with the differentiation, metastasis and prognosis of several kinds of tumor.In the process of tumor invasion and metastasis, BM is the natural barrier which is first met in the process of tumor cell metastasis. Only if dissolving this series of tissue barrier, the tumor can form a pass way for moving. ColIV is the basic fracture bone of BM and also the specific component which can be special symbol. The defect or incompletement of ColIV is the first condition for the invasive development of the tumor in the tissue.As already proved, tumor in head and neck first metastasize to lymph node belong to this area, then overwhelm this barrier and metastasize to the part far a-way.
