Objective: To study the treatment for lower cervical spinal fractures and dislocations without spinal injury.Methods: Among the twenty-three cases of lower cervical spinal fractures and dislocations without spinal cord injury, eight were treated with conservative method and the other fifteen with anterior decompress- sion, reduction, autograft and armor plate fixation.Results: The long term clinical therapeutic effects of the 8 cases with conservative treatments were not satisfactory. Three months after injury, 3 cases with conservative treatments were treated with anterior surgery again. In the fifteen cases with surgery, clinical manifestations were improved, with solid fusion obtained within 3 months, and there were no plate or screw break or loose. Long-term effect was positive.Conclusion: Anterior approach is preferred to lower cervical spinal fractures and dislocations without spinal cord injury so that it is feasible to get excellent maintenance of intervertebral heights and physiologic curves, meanwhile the cervical spine can be stabilized,cubage of cervical canal in the injured segments can be resumed or enlarged and secondary injury to spinal cord will be prevented.
