In recent years, extracapsular cataract extraction (ECCE) and intraocular lens (IOL) implantation play the main role in cataract surgery. Posterior capsule opacification (PCO) is a significant long-term complication that affects up to 50% of patients within 2 months to 5 years after ECCE, that leads to visual acuity decrease. The incidence of PCO is far greater in patients with congenital cataract than in those with senile cataract and is thought to reflect the higher proliferative capacity of lens epithial cells (LECs) in the young. In PCO, residual LECs in the anterior and equatorial regions of the lens capsule after surgery proliferate and migrate onto the previously acellular posterior capsule. These cells form a multiplayer, take on a fibroblast-like appearance, contract, and secrete extra cellular matrix. The resulting fibrocellular membrane causes significant visual loss when it reaches the visual axis, necessitating a posterior capsulotomy, which is also associated with several ocular complications.The high incidence of PCO led to the concept of performing a primary posterior capsulotomy and anterior vitrectomy at the time of cataract extraction and IOL implantation in children and associate with pharmacological agents. In these study, to investigate the growth characteristics of bovine epithelial cells by in vitro three dimensionally, cultured bovine lens capsule in the drug prophylactic-therapitic,we observed the effect of Matrine, Diclofenac Sodium, Pranoprofen on the proliferation of BLECs.1. Investigation of three-dimensionally cultured bovine lenscapsuleMethod: Use the PMMA(polymethyl methacrylate) made the supporter.The anterior capsule were separated and cultured in condition of 37, 5% CO2 by suspending on the special support for 6 weeks. In time, 3d, Iw, 2w, 3w, 4w, 5w, 6w, the proliferation of BLECs cultured from the lens anterior capsule was observed by Giemsa staining and inverted microscope.Results: The fresh anterior capsule is transparent without cells proliferation.BLECs from all lens capsule proliferated after 3 days. 1 week later,the whole capsule apparensed cloudy opacification,a lots of lens epithelial cells proliferated in the center of the capsule .2 weeks later,except the central cloudy opacification,there were dot-blot opacification.The lens epithelial cells apparensed stratified proliferation. After 3 weeks, the dot-blot opacification apparensed more in the central area, LECs were obviously apoptosis and began to exfoliate. After 6 weeks,all the cells exfoliated, the lens capsule became transparent.2. The inhibition effects of Matrine, Diclofenac Sodium andPranoprofen on BLECsMethod: BLECs were isolated and cultured. The passage BLECs were placed in 24-well tissue culture plates and incubated for 24 hour or 48 hours, respectively. (2) Growth of cells was observed, after 24 hours ,48 hours, absorbency of the cells was tested by using Giemsa staining and colorimetry and the median lethal dose (LD50) was calculated respectively.Results: Matrine,Diclofenac Sodium and Pranopofen significantly suppressed the proliferation of BLECs in vitro, showing dose-dependent and time-dependent manner. The calculated LD50 of Matrine, Diclofenac Sodium and Pranoprofen for BLECs was187.50, 119.14, 79.04 mg ?L-1.3. Conclusion(l).The prolifertation and apoptosis of the lens epithelial cells are obvious in vitro suspended culture,but the differentiation toward fibroblasts is not considerable.(2). This culture model can explain the clinical phenomenon caused by retaining the anterior capsule of lens during cataract surgery.(3).Matrine,diclofenac sodium and pranoprofen can suppressed the proliferation of BLECs in vitro,all of them can be used in inhibiting the after cataract.
