The Mechanism Study Of YanShen Capsule To Retard The Remnant Kidney Fibrosis

Objective:To clarify the mechanism of YanShen Capsule which retards progressive fibrosis of the remnant kidney. Methods:120 adult Wistar rats were randomly divided into 6 groups,20 rats in each groups. Each group has two time points,10 rats in each time point. Rats of group A were sham-operated, other five groups ( group B, C, D, E, F) were 5/6 nephrectomized. Group C, D, E were given low, medium and high concentration of YanShen capsule (which was made of chinese herbs by professor Yuan Fahuan). Group F were given comparative drug Shen Shuai Ning Capsule (which was produced by the Yunnan Lixiang Yaoye).Drugs were perfused into stomach every day. When day 15 and 90 after operation, optical microscope, trans-electron microscope, morphological quantitation, biochemical index (BUN,SCr,Hb,Angâ…¡), immunohistochemical (TGF, Col IV, MMP-9, Fas, FasL, PCNA) and TUNEL were observed. Results:1. It was found that the group B, Serum creatinine (SCr) ,blood urea nitrogen (BUN) and angiotensin â…¡(Angâ…¡) levels were obviously higher than group A., but hemoglobin (Hb) was lower than group A. For group C, D, E, and F, these indexs were better than group B.2. When 90 days after operation, in group C, D, E, and F, the fibrosis extent of the remnant kidney were obviously lower than group B. Under trans-electron microscopy,there are a lot of apoptosis of proliferative mesangial cells in group C, D, and E. For group A and B, there were little apoptosis. 3. When 15 and 90 days after operation, in group C, D, E, and F, the expression of transforming growth factor-(1(TGF-(1), type â…£ collagen (Colâ…£), proliferating cell nuclear antigen (PCNA) were significantly lower than group B. For these indexes, there were not obviously differences between group C and F.4. When 15 and 90 days after operation, in group C, D, E, and F the expression of Fas, Fas ligand (FasL), matrix metalloproteinase-9 (MMP-9), and TdT mediated X dUTP nick end labeling (TUNEL) were obviously higher than group B. For these indexes there were not obviously differences between group C and F. 5. The proliferative cell index in group C, D, and E was lower than group B. The apoptosis cell index in group C, D, and E was higher than group B., It was similar in group C and F.Conclusion:1. Under certain experimental conditions, YanShen Capsule inhibited fibrosis and protected function of the remnant kidney significantly. YanShen Capsule, which inhibited glomerular sclerosis and interstial fibrosis, had a dose dependent protective effect.2. The effects of YanShen Capsule on inhibiting the fibrosis may be resulted in inhibition of cell proliferation and promotion proliferative mesangial cells apoptosis in the remnant kidney, which decreased ECM synthesis and increased its degradation.
