Object: The purpose of this study was to indicate more accurate diagnostic methods with ultrasound imaging in the diagnosis of salivary glands tumors. In this study, 2DUS,CDFI, CDE and PWD were used in the research on the rumors of salivary glands. The roles of each method in the diagnosis of benign or malignant tumor were analyzed.Materials and Methods: 142 patients (Male: Female= 80: 62) with primary salivary glands tumors were included, In these cases, 95 were benign and the others were malignant. All the patients were examined by Colorful Doppler Ultrasound Equipment (GE LOGIQ400, America) and the rumors were diagnosed as benign or malignant mass according to their 2DUS, CDFI, CDE characteristics. The results were compared with the surgical and pathological diagnosis. In addition, the CDFI and the CDE expression in one same blood flow of the tumor were compared with the hemodynamic index of tumors.Results: The borders, morphology, internal echo and posterior echo of tumors were well showed by 2DUS. The internal echo and posterior echo displayed obvious difference between the benign tumors and the malignant ones. They indicated a high specificity (87.4%) and a low false positive (12.6%) in diagnosisof benign and malignant tumors. Also the imaging overlap of the benign tumors and the malignant ones was observed, especially between the progressive benign tumors and the low malignant ones. And the accuracy was 81.7% in diagnosis of benign and malignant tumors.CDFI can clearly demonstrate the blood flow of the tumor. Results showed that the demonstrative rate of blood flows were higher in malignant tumors than in benign ones. The tumors showed different blood flows and more overlap were found in middle volume. The blood inflow of tumors showed valuable indication on the diagnosis of benign and malignant tumors: there is significant difference between the blood flows of malignant and benign tumors (p<0.01), but the Warthin's tumor was excluded (P>0.05) . Of all the other benign tumors, 78.8% showed blood inflow of grade I or II", while 80.0% of malignant tumors showed grade II orIII.The hemodynamic index of tumors measured by PWD showed that there was significant different PSV between benign and malignant tumors. The mean PSV was higher in malignant tumors (p<0.05), but RI and PI showed no significant difference. When PSV>55cm/s, the tumors should be malignance suspected.The demonstrative rate of blood flows by CDE was higher than CDFI. To one blood vessel, CDE showed a longer one than CDFI and it can display its shape. But there was no significance between them in the diagnosis of benign and malignance.Compared with the post operation pathological diagnosis, the ultrasound diagnosis indicated 89.3% sensitivity, 90.5% specificity, 10.6% false-negative rate, 9.5% false-positive rate, 90.1% accuracy, 94.5% negative prior-announcement and82.4% positive prior-announcement.Conclusions: Ultrasound imaging is an effective method in the diagnosis of salivary glands tumors. 2DUS plays important roles in the detection and the primary classification of tumors. CDFI can clearly show the form of blood flows and the blood supply and thus the preliminary assessment of the tumors nature can be made. As for the velocity of blood flows, it showed high specificity but low sensitivity. CDE is superior to CDFI in the demonstration of blood vessels. The results indicated that combined use of these methods could enhance the diagnosis accuracy, which is of great importance in the clinical diagnosis and the final treatment. |
