[Objective]: To study the effect and security of amnioinfusion(AI) on labor complicated by moderate or severe meconium-stained amniotic fluid.[Methods]: 88 delivery women complicated by moderate or severemeconium-stained amniotic fluid were chosen, given common treatment as sucking oxygen, lying on left side and dropping intravenous with vitamin C added to glucose. With the consent, 44 women were treated witn AI(Group A), while the other 44 cases received common treatment only(Group B). Another 44 delivery women with clear amniotic fluid were also chosen as control group(Group C), which obtained no any treatment.[Results]:1. There were significant differences of amniotic fluid index (AFI) after AI inGroup A. The variable deceleration (VD) rate was similar between Group A and B, and both groups got the VD' remission after their treatments, but the remission rate of VD was higher in Group A than that of Group B.2. The umbilical venous blood gas analysis of 1 minute newborns: the PH value in Group B was significantly lower than that in Group A and C. And the BE absolute values in Group B was higher than that in Group A and C. But no difference was found between Group A and Group C.3. After AI, the amniotic fluid in 34 cases in Group A became clear or gently stained, and 24 cases got spontaneous delivery, with no newborn' MAS or distress of respiration. The rates of meconium below the vocal cords,MAS and Apgar scores ≤ 7 were similar between Group A and B.4. The cesarean section rate of Group A is lower than that in Group B. There were no significant differences in total and the second stage of labor duration.5. There were no significant differences of positive bacteria numbers in culture medium, average WBC and CRP of the pregnancies 3 days postnatally. The puerperal morbidity was similar in three groups.[Conclusions]:1. AI can improve the amniotic fluid index in labor and reduce the fetal heart monitor' variable decelerations efficiently, which is prefer to common treatment, such as sucking oxygen, lying on the left side and dropping intravenous with Vitamin C added to glucose.2. After AI, most of the patients' amniotic fluid transferred from moderate or severe meconium-stained to clear or slightly stained. AI can reduce the cesarean section rate but does not effect the total and the second stage of labor duration.3. AI can improve the fetal anoxia status of moderate or severe meconium-stained amniotic fluid.4. AI does not improve the puerperal infection rate.
