1. BackgroundGiomus tumor is a rarely genuine tumor originated from glomus body attaching to tiny arteriovenous anatomotic branches,and caused by the induction of trauma,chronic pain characterized by tenderness to palpation,sensitivity to coolness and unelicited pain is its mainly clinical symptom[3,4],however,the pathophysiological cause of pain is unknown[5], Studies nearly past 20 years concentrated in histogenesis and the relationship between multiple glomus tumor without chronic pain and familial heredity,while there have been no reports about the research in pathophysiology of chronic pain,especially up to now,no effective treatments for multiple glomus tumor with chronic pain,The study observed the demonstration of SP,NF between glomus tumors with chronic pain and control samples by means of immunohistochemical technology and computer graph analysis system in order to discuss the process of chronic pain, in addition,Correlation between immune expression of pain symptomatic groups and level of pain was analyzed. All the researches mentioned above are hoped to supply theoritical base for the therapy of chronic pain in glomus tumor and other traumatic chronic pain.2. ObjectiveTo observe the expression and distribution of substance P(SP), neurofilament (NF) in glomus turners with chronic pain,and discuss the process of chronic pain as well as the relationship with degree of pain.3.Materials and methods27 patients diagnosed glomus tumor with chronic pain were enrolled ascase group, and divided into light pain symptomatic group (LPSG)(n=12) and severe pain symptomatic group (SPSG)(n=15) according to clinical manifestation. Control group were also enrolled by 30 patients with amputated extremities or hands after trauma. Immunohistochemical methods were used to determine the expression of SP,NF which were detected quantitatively by computer graph analysis system too.4.ResultsPositive expression and distribution of SP,NF were in all three groups,expression level of SPSG was the highest [Grayscale Valuesp(143.3+ 7.5),Grayscale Valuer (167.7+4.4)], LPSG followed [Grayscale ValueSp(156.2+8.2),Grayscale ValueNF (194.8+4.0)], control group was the third[Grayscale Valuesp(208.2+16.6), Grayscale ValueNF (225.1+8.3)];The difference of expression level among three groups was significant [SPSG vs LPSG (Psp=0.002,PNF<0.0001),SPSG vs CG(Psp<0.0001,PNF<0.0001),LPSG vs CG(Psp<0.0001,PNF<0.0001)].5.ConclusionSP is the mainly afferent pain transmitter in the process of chronic pain in glomus tumor,and maybe there exists correlation between immune expression of SP in pain symptomatic groups and level of pain,NF plays an important role in pain-transmitted activities.
