Objective To investigate the expression of P27Kip1, E-cadherin in breast carcinoma in relation to biological behavior and prognosis.Methods Expression of P27Kip1 , E-cadherin protein was detected by immunohistochemical technique in 51 breast invasive ductal carcinoma specimens. The relationship of P27Kip1 and E-cadherin expression to clinicopathological characteristics and postoperative survival time was statistically analysed.Results (1) Of the 51 cases, P27Kip1 expression was high in 18 (35.3%) and low in 33 (64.7%) respectively. P27Kip1 expression level was associated with tumor size, axillary lymph node status, TNM stage, histological grade, local recurrence and distant metastasis, and c-erbB-2 expression. Univariate survival analysis showed that the postoperative 5 -year survival of patients with high P27Kip1 expression was significantly higher than those with low P27Kip1 expression. Cox proportional hazard model analysis showed that P27Kip1 expression level was an independent prognostic factor in breast carcinoma. (2) The high and low E-cadherin expressive rate was 41.2% (21/51) and 58.8% (30/51) respectively. E-cadherin expression level was correlated with tumor size, axillary lymph node status, TNM stage, histological grade, local recurrence and distant metastasis. Univariate survival analysis demonstrated that patients with high E-cadherin expression showed better postoperative 5-year survival. However, by Cox multivariate analysis E-cadherin expression level was not found to have independent prognostic significance. (3) No agreement was found between P27Kip1 expression and E-cadherin expression.Conclusion (1) P27Kip1 expression is significantly associated with tumor development, progression and prognosis in breast carcinoma, so may serve as a valuable tumor marker and independent prognostic indicator. (2) Reduced E-cadherin expression in breast carcinoma promotes malignantprogression, indicates dedifferentiation and intensive invasive biological behavior. E-cadherin expression level is an important prognostic factor in breast carcinoma. (3) The relationship of P27Kip1 expression to E-cadherin expression is deserved further investigation.
