Objective: To investigate the changes of plasma concentrations of vWF (von Willebrand Factor), ET (Endothelin) and the expression of GPIIb/IIIa receptor (CD4)) before and after PCI (percutaneous coronary intervention) in the patients with angina pectoris.Methods: 40 patients with unstable angina pectoris (UAP) and 38 patients with stable angina pectoris (SAP) were enrolled. Serial venous blood samples were obtained immediately before and 30 min, 4, 12, 24,48, 72 hours, 7days after PCI. Plasma concentrations of vWF and ET were determined by enzyme linked immunoassay. The expression of CD4) was assessed by flow cytometry. Meantime, blood samples of 20 normal subjects demonstrated by CAG (coronary angiography) as control group Were" drawn immediately before and after CAG.Results: The plasma concentrations of vWF, ET and the expression of CD4iWrere no significantly higher in nomal controls after CAG than those before CAG.The plasma concentrations of vWF, ET and the expression of CD4i were significantly higher in UAP andSAP groups after PCI than those before PCI.The peak levels of vWF (279.87 + 37.56)%, ET (33.78 + 10.76) ng/ml and CD41 (288.76 + 48.21)MFI were at 4 hours, 30mins and 4 hours respectively in UAP group after PCI.The plasma concentrations of vWF, ET and the expression of CD4] in UAP patients, with type C were significantly higher than those with type A or B before and after PCI. These changes were not seen in SAP group.The increases of the plasma concentrations of vWF, ET and the expression of CD4] in UAP patients were significantly higher than those in SAP patients after PCI.There was a positive correlation ( r = 0.678, p <0.01) between the increases of vWF and CD41 in all patients after PCI.There was a positive correlation ( r = 0.745, p O.01 ) between the increases of vWF and ET in all patients after PCI.Conclusions: There are different degrees of the endothelium damage and the activity of coagulation system between UAP and SAP groups before PCI.The increases of vWF, CD4] and ET are closely correlated to the stability of coronary artery lesion and the degree of atherosclerotic plaques.The interaction among vWF, CD4] and ET aggravated the endothelium damage and the abnormal activity of coagulation system after PCI.The assay of vWF, CD4) and ET before and after PCI may offer rationale for preventing the early and long-term complications.
