| The research is to design and explore the power reconstruction for the broken anterior cruciate ligament (ACL) and posterior cruciate ligament (PCL) of the patients with half or complete dislocation of the knee joint caused by the break of ACL and PCL. The pivotal technology of the power reconstruction is to design the power reconstruction for the ACL by using the semitendinosus muscle of the postero-medial aspect of the thigh and using the peroneus longus muscle for the PCL, according to the anatomical characteristics of the mass muscles around the knee joint without reducing the muscles' effect on the functions of limb. The method was applied on 12 cases (8 males and 4 females) including the power reconstruction for the ACL with the semitendinosus muscle, the power reconstruction for the PCL with the peroneus longus muscle, the repair of the medial collateral ligament, the improved pes anserinus forming, the repair of the lateral meniscus and the collateral ligament inOrthopedic Surgery Department, Jilin Central Hospital from March, 1991 to March, 2001. The knee joint were kept straight by plaster cast for 4~5 weeks after operation. Then take off the plaster cast and try to do functional train.By investigation, the knee joint was stable, the patients could walk evenly, the tests of antero-drawer and postero-drawer and the tests of medial and lateral press were all negative. The treatments were successful. As for the reconstructive material, the muscles and tendons are active which under the control of the nerves and supplied by blood. They are put in the original place of the cruciate ligaments according to anatomical structure and have the characteristics of stabilizing the knee joint. It's not only the power reconstruction, but also the intensity and the endurable drawing force are similar to the original one. They are planted in the bony tunnel firmly and connected with the bone tightly. It has the function of biological self-fixation.The effect on the function of the limb was splendid after taking away the semitendinosus muscle and peroneus longus muscle, and there were not any significant side-effect. As we know, the peroneus longus muscle doesn't involve in the stabilization of the knee joint, therefore, the original structure of the knee joint could not be crashed by cutting off it. The operation is actually a kind of transplantation of muscle and tendon and it doesn't affect the control of thenerves and the supply of blood to the muscle and tendon.It is easy to get material, and the muscle and the tendon are long enough to reconstruct the ACL and the PCL.The disadvantage of the operation is that there are many cuts involved.The operation offers a new way to repair and reconstruct the ACL and the PCL to the most orthopedists. Compared with the same kind of operation, this method is very advanced in the nation. |
