In clinical cases analysis, people always retrieve the cases and record the information in handwork. They repeat a great deal of unnecessary work. With the development and popularization of multimedia technique, computer becomes an indispensable tool in our work, study and scientific research. To manage the clinical information by database can obtain twice the result with half the effort. We found Access97 Data Base Management System(DBMS) had great function and easy to master. We study all 1374 cases of colorectal carcinoma who had undergone surgeiy between 1975 and 1999 in NanFang hospital, and built up Access97 database. Taking advantage of the database, we analyzed the correlations between clinical and pathological factors and colorectal carcinoma lymph node metastasis, and the internal relation among them. 1. Correlation analysis of many factors causing lymph node metastasis of colorectal cancer To search the factors related with the lymph node metastasis (LNM) of colorectal cancer. Method The study included 1374 patients with colorectal carcinoma who had undergone surgery between 1975 and 1999 in NanFang hospital. We analysis all the cases and up build Access database, find out the relationship between every factor and LNM, discriminant analysis and multiple regression with the aid of SPSS9.O/PC+ software. Results The lymph node metastasis rate of age <30 and >50 was 51.32% and 31.33% respectively. The rates of LNM rise following with the increase -4- of depth and circumference of penetration. The difference between metastasis rates in both conditions was remarkably significant. The rates of LNM at protruded type, ulcerative type, infiltrating type and colloid type was 30.92%, 39.67%, 42.11%, 42.86%,respectively. The rates of LNM at ademocarcinoma, mucoid carcinoma, signet-ring cell carcinoma was 33.3%, 49.6%, 61.1%. LNM occurred significantly less often in Well-differentiated cancers than that of bad-differentiated. Tumor size and serum CEA level before operation were not significant predictive features for LN7M. Conclusions The main factors related with LNM of colorectal carcinoma was age, circumf抏rence and depth of penetration, gross type, pathological type, differentiated degree, and growth type. Tumor size and serum CEA level before operation were not significant predictive features for LNM. 2. Research of the relationship between related factors with lymphs node metastasis of colorectal cancer This study was proposed to find out the relationship between related factors with lymph node metastasis of colorectal cancer. It may provide an important basis in diagnosis and treatment of colorectal cancer. The study included 1374 patients with colorectal carcinoma who had undergone surgery between 1975 and 1999 in NanFang hospital. We analysis all the cases and up build Access database, find out the relationship between every factors, discriminant analysis with the aid of SPSS9.0/PC+ software. On univariate analysis, six variables had a significant association with circumference of penetration, these variables were: age; depth of penetration; growth type; differentiated degree; pathological type and tumor site. In group of age <30, the rate of mucoid carcinoma, signet-ring cell carcinoma and low differentiated adenocarcinoma are greater than other groups. The rate of carcinoma spread involving a free...
