ObjectivesTo find out the occurrence of progressive hepatic fibrosis, related influential factors.Methods From July 2014 to October, a cross-sectional study has been used.4 hospitals in 4 provinces/municipalities have been chosen.More than 18 years and diagnosed as chronic hepatitis C patients were surveyed by a self designed questionnaire. The main content of questionnaire contained general demographic characteristics,the epidemiology of hepatitis C virus related factors of history and behavior, the prevalence of diabetes mellitus and non alcoholic fatty liver,treatment,the cognition of hepatitis C, medical burden.Researchers looked up medical records and made a supplement of liver hardness value, with HIV or HBV co-infection.Entered data with Epidata and analyzed data with SAS.The general demographic characteristics is described by the proportion. Used logistic regression to analyze major related factors of progressive fibrosis.Results1. General demographic characteristics:A total of 359 chronic hepatitis C patients including 171 males(47.6%) and 188 females(52.4%) were under investigation, with the average age of 55.1±11.9.Most patients were ethnic Han. Most of them were with lower than junior education,married or cohabiting accounted for the majority(91.6%). Farmers and retired personnel were more. The monthly income of most of them was less than 3000 yuan(77.2%).2. Hepatitis C virus related factors of behavior:124 out of 359(34.5%) had a history of blood transfusion,65.5% people did not have a history of blood transfusion.62.1% did not have the history of drinking.2.8% alcohol consumption was less than 10 years.4.2% alcohol consumption was between 10 and 19 years.30.9% alcohol consumption was more than 20 years.The people with daily alcohol intake of more than 50 grams accouted for 28.4%.66.4% did not have the history ofsmoking.1.4% tobacco consumption was less than 10 years.3.1% tobacco consumption was between 10 and 19 years.29.2% tobacco consumption was more than 20 years.The people with daily tobacco intake of more than lpack accouted for 12.0%.11.7% had diabetes mellitus and 18.9% had non alcoholic fatty liver. The body mass index of 37.8% people were more than 24.4.7% were with HIV or HBV co-infection.44.8% did not take the antiviral treatment.43.2% did not know the knowledge of hepatitis C.3.The Occurrence of Progressive fibrosis:218 patients with progressive fibrosis, theincidencewas 60.7%.129 out of 218(59.2%) had fibrosis cirrhosis.4.Major related factors of progressive fibrosis:According to single factor logistic regression analysis, age≥40, male, less than Junior high school education, farmer, age at infection≥25, non blood transfusion infection,drinking time, alcohol consumption≥50g/d,smoking time,smoking≥ 20 cigarettes/day, Diabetes mellitus, Nonalcoholic fatty liver disease, not received antiviral treatment, HIV/HBV coinfection,not know the knowledge of hepatitis C.appeared as the main factors that influencing the level of progressive fibrosis among the patients with chronic hepatitis C in hospital. Data from the multivariate logistic regression analysis that factors as:age≥40, alcohol consumption≥50g/d,not received antiviral treatment,HIV/HBVco-infection, were associated with high incidence of progressive fibrosis.Conclusions1. This study indicated that theincidence of progressive fibrosis was high. Paying attention to hepatitis C patients and strengthen the detection of liver fibrosis, early treatment and control the progress of liver fibrosis.2. Age≥40, daily alcohol intake of more than 50 grams, HIV/HBVco-infection, not received antiviral treatment,were associated with high incidence of progressive fibrosis. Promoting early treatment of hepatitis C and other diseases, strengthening the detection of liver fibrosis in age more than 40 years and drinking behavior intervention may contributed to control the the progress of liver fibrosis.
