Study On The Intervention Effect Of Interventional Intervention And Interventional Nursing Measures Of Oral Tracheal Intubation In ICU Adult Patients

Purpose This study was designed to describe and analyze the current status of the self-extubation in orally intubated adults in the intensive care unit and also to form care bundles on the basis of evidence-based nursing and combination with clinical practice and also to apply strategy to practice so as to reduce the incidence of unplanned extubation.Methods Investigated the current status of the self-extubation in orally intubated adults in a third-grade class-A hospital intensive care unit from January- December 2014 by using the method of convenience sampling; On the basis of the evidence-based nursing combined with clinical practice and the results of analysis, this study formulated care bundles of unplanned extubation;Applied care bundles to intervention group in January-march, 2015 in the intensive care unit, applied conventional nursing to control group in the same intensive care unit at the same period in 2014, compared the incidence of unplanned extubation of two groups.Results(1) Total 26 UEs occurred during the study and they all 21 UEs occurred during day shifts,16 UEs occurred with nurses who had less than one years’ experience in ICU.The incidence of UE was 4.54% and the incidence density of UE was 1.38 per 100 intubation days.Multivariable regression analysis revealed that the categories of the Richmond Agitation-Sedation Scale were associated with a risk of UE.. TISS- 28 points and the incidence of unplanned extubation have no statistical significance(2) The incidence of unplanned extubation in two groups was seperately 0.5 per 100 intubation days for the intervention group and 2.1 per 100 intubation days for the control group and there was statistical significance between the two groups.Conclusion The categories of the Richmond Agitation-Sedation Scale were associated with a risk of UE;Care Bundles were the ideal nursing methods. The strategy can effectively reduce the incidence of unplanned extubation with correct application in the clinical practice and continuous improvement.
