Objective : To discuss the effect of extending subdural drainage time in the treatment of severe traumatic brain injury after craniotomy.Methods: 116 patients with severe traumatic brain injury after craniotomy were randomly divided into treatment group and control group. Patients in treatment group were treated with subdural drainage tube removed in the first four days after craniotomy,and the control group patients were routinely on the first two days with subdural drainage tube removed after craniotomy.Results:①the CSF RBC <100×106 / L time and TP amount <0.8g / L time of the treatment group were significantly shorter than those of the control group(P <0.05);② 1 week after injury, the average ICP, ICP increased duration, mannitol use of the treatment group were significantly lower than those of the control group(P<0.05); ③The occurrence rate of hydrocephalus and cerebral infarction of the treatment group were significantly lower than those of the control group(P<0.05); ④The disability rate and The fatality rate of the treatment group were significantly lower than those of the control group(P <0.05).Conclusion : t Extending subdural drainage time can accelerate clearance in patients with bloody cerebrospinal fluid, Auxiliary reduce intracranial pressure, reduce the amount of mannitol, reduce complications, significantly reduce the rate of severe disability and mortality, improve patients’ outcomes.
