| Purpose:Chronic urticaria is a common clinical disease in Department of dermatology, nearly all age groups can beaffected, the longer disease duration and easy to repeated attacks.The intense itching often seriously affect the patient’s quality of life,and the majority of patients of chronic urticaria are often could not found the pathogenic factors, the current linical treatment with antihistamines to control symptoms, but only temporarily effective, does not prevent recurrence, most patients relapse after stopping drug.Therefore to explore the pathogenesis of chronic urticaria and new effective treatment become an important subject at home and abroad in recent years,traditional Chinese medicinehave the opinion that theessential reasons of the disease is endowment intolerance and exterior deficiency,or a long illness hurt the Qi and blood,deficiency of Qi and blood caused, so the key of the therapy is reinforcing qi and nourishing blood,Yi qi yang xue qufeng decoction is the Professor Zhang Yongxi’sprescriptionin our department,it commonly used in clinical treatment of chronic urticaria,its source in classic prescription of Chinese Angelica Decoction addition and subtractionof changes.This study is to observe the clinical efficacy of Yi qi yang qu feng Decotion in the treatment of qi-blood deficiency of chronic urticarial. And to test the level of serum Ig E and the quality of life(QOL) before and after treatment, learn about the effect of Yi qi yang xuequ feng Decoction to them. To provideidea and basis for possible reliable treatment.Method:A randomized control clinical trail, 108 cases of patients with chronic urticarial which had been qualified, were randomly divided into the treatment group(group A) and control group(group B), each group of 54 cases,the group A with Yi qi yang qu feng Decotion 100 ml, 2 times a day orally; Both thetwo group with Ebastine tablets 10 mgorally per day.Two groups of patients were treatedwith 4 weeks as a course, two courses were observed, and followed-up two months after the end of the study.Observing the clinical effect and relapse of the two groups,and the changesof serum Ig E and QOL.Results: 1. At the end of the first course of treatment,the Urticarial Activity Score(UAS7)of group A was 21.95±6.46, group Bwas 23.22±5.66, no significant difference between the two groups of score(t=-1.138, P>0.05). QOLscore of group Awas 52.10±13.42, group Bwas 55.17±10.35, differences between the two groups has nostatistical significance(P>0.05), Namely the two groups had no significant differences in efficacy and quality of life at the end of the first course of treatment. 2. At the end of the second course of treatment, the UAS7 SCORE in group A was 11.724±5.86, and 20.363±4.60 in group B, there were significant differences between the two groups(P<0.01), group A was lower than that of group B(The disease activity of group A was lower than that of group B).The QOL score of group A was 26.50±11.87, while group B was 39.11±13.10,the differences were significant(P<0.01), group A was lower than that of group B(The quality of life of group A was better than that of group B).The level of serum Ig E of group A was 294.71 ± 45.59IU/ml,group B was346.16 ± 38.78IU/ml,the differences were significant(P<0.01),Ig E level in serum of group A lower than group B. 3. At the end of the treatment, evaluated by SSRI index, there were 38 effectivecases in group A, and 14 invalid cases, the total efficiencywas 73.1%, group Bwas 17 effective cases, 32 invalid cases, the efficiency was 34.6%, two group of comparison by chi-square test, χ2=13.39, P<0.01, the efficacy of group A is better than group B. 4. Both the UAS7 and QOL scores of the two grouswere increased in the two months of followed up as time goes on, but overall the scores were lower in group A than in group B. According to the SSRI index to evaluate the rate ofrecurrence, group A was 13:38,the recurrence rate was 34.2%, group B was 17:15, the recurrence rate was 88.2%, the recurrence rate of group B is higher than that of group A(P<0.01).Conclusion: 1. Yi qi yang qu feng Decotion can effectively improve the clinical symptoms of patients with Qixue liangxu of chronic urticarial. 2. Yi qi yang qu feng Decotion can reduce the disease activity of chronic urticarial, improve the quality of life in patients with chronic urticarial. And with the course of increasing, the effect is more obvious. 3. Yi qi yang qu feng Decotioncan reduce the levels of serum Ig E,and with the course of increasing, the effect is more obvious. 4. The long term efficacy and the recurrence rate of the group of Yi qi yangxue qufeng Decoction and ebastine was significantly better than that of simple ebastine group, and the drug dosage of ebastine could be reduced. |
