The theory of treatment based on spleen, which always guides clinicalpractice effectively, is an integral part of basic theories of Chinese medicine.A Guide to Clinical Practice with Medical Records written by Ye Tianshi contains2576medical records,which is of great significance to the development ofinternal medicine of traditional Chinese medicine, and embodies the view ofattaching great importance to the spleen and stomach. Ye Tianshi was deeplyinfluenced by Treatise on Febrile Diseases written by Zhang Zhongjin and Treatiseon the spleen and stomach written by Li Gao, and proposed the theory ofinterdependence between drying and moistening and interdependence betweenascending and descending of the spleen and stomach. Because of different physicalproperties of the spleen and stomach, it was written that major therapeuticmethods were raising spleen yang, nourishing stomach yin and tonifying spleenyin. Through collecting and studying medical records on Chest Stuffiness andPains, Spleen-Stomach Diseases and Blood Syndromes in A Guide to ClinicalPractice with Medical Records,this paper summarized their etiologies andpathogenesis, therapeutic methods and representative formulas. The majorpathogenesis of Chest Stuffiness and Pains, Spleen-Stomach Diseases and BloodSyndromes was dysfunction of the spleen in transportation and transformation.The major therapeutic method of Spleen-Stomach Diseases was to replenishing qito invigorate the spleen and to regulate qi-movement,which commonly appliedSijunzi Decoction. The way to treat Chest Stuffiness and Pains was toreplenishing qi to invigorate the spleen and to warm phlegm and fluid, whichcommonly applied Linggui Zhugan Decoction. Because of the wide range ofSpleen-Stomach Diseases, treatment and prescriptions varied with differentsyndromes. The way to treat Blood Syndromes was to tonify qi to control blood,which commonly applied Guipi Decoction. The academic thoughts on treatment basedon spleen of Ye Tianshi provide a theoretical basis and reference for clinical diagnosis and treatment.
