The study intends to explore a mechanism to manage chronic diseases through the mode of medical association, include building the system of hypertension management, building the mechanism of hypertension management, the evaluation index selection and data acquisition, the extension of the mechanism to manage chronic diseases through the mode of Medical Association, in order to realize the need that hierarchical diagnosis and treatment for different patients with essential hypertension, increase accessibility of medical service, reduce the economic burden of patients and medical institutions, improve the patient’s healthy level and quality of life. Provide reference to the extension of the mechanism to manage chronic diseases through the mode of medical association.To achieve the research purposes, in this study, I have consulted a large number of domestic and foreign literatures, through the research on the USA, Britain, Singapore and other countries of the mechanism to manage chronic diseases through the mode of medical association to understand the domestic and foreign research situation about it. In the form of questionnaire to continuously track and record the data of the hypertension patients who are under the service of Laiguangying community health service centers which belongs to China-Japan Friendship Hospital and through the Xikang health management platform to upload the data to realize the sharing of medical information. In the end, through professional and system collection, arrangement, summary and using statistical methods for statistical analysis to obtain the effect of management and problems. With depth interviews and expert meeting while combining with the research of related fields, to explore the effect of the mechanism and the reasons due to the problem. And then provide practical reference for establishing the mechanism to manage chronic diseases through the mode of medical associationThrough the study, I found that with the mechanism of managing chronic diseases (hypertension) through the mode of medical association, the medical expenses and treatment time of patients with hypertension were reduced. And it improved the availability of quality health resources and the utilization rate of health resources in primary health institutions, and the satisfaction of community hypertension patients prevalence in the primary medical institutions. In the same time, to some extent, alleviated the pressure of large hospital outpatient hypertension,. Data displays that,1.The effect of the information technology and the grading treatment group were both better than that of the traditional one. Reflected in the changed of mean of the low pressure was bigger than the follower, and it had statistical significance. But the difference between the information technology and the grading treatment group was not significant, so that we could not hold that the information technology is better than grading treatment group.2. The changes of patients’ quality of life in the information technology group and the grading treatment group were both better than that of the traditional one. Reflected in the changed of mean of the evaluation was bigger than the traditional group, and the result had statistical significance. To compare the every two groups from the data of the three groups, it showed that patients had better life in the information technology group than the traditional group, grading treatment group was also better than the tradition group, and both of them had statistical significance. But the difference between the information technology and the grading treatment group was not significant, so that we could not hold that patients in the information technology group had better life than the grading treatment group.3. The medical expense of patients in the information technology group and the grading treatment group were significantly reduced compared with that of the traditional one. And there was the statistical significance. To compare the every two groups from the data of the three groups, it showed that the expense of the information technology group was smaller than that of the traditional group, grading treatment group was also smaller than the tradition group, and the information technology group was smaller than the grading treatment group.In addition, all of them had statistical significance.4. The treatment time of patients in the information technology group and the grading treatment group were significantly reduced compared with that of the traditional group, and it had the statistical significance. To compare the every two groups from the data of the three groups, it showed that the time of the information technology group was lower than that of the traditional group, grading treatment group was also lower than the tradition group, and the information technology group was lower than the grading treatment group.In addition, all of them had statistical significance.Through the comparative study of the three groups, the effect of the information technology group due to the following aspects. The main reasons are bad compliance, effective mentality impatience, the lack of the understanding of hypertension especially the complications of hypertension which leads to neglect of the disease.According to the problems found in the study, there are a few suggestions for the extension of the mechanism to manage chronic diseases through the mode of medical association:1. Encourage the establishment of medical association which consists of three general hospitals, two general hospitals and primary health care institutions, then establish the mechanism to manage chronic diseases through the mode of medical association based on information platform, improve residents’recognition of the grassroots medical institutions, realize internal referral and classified diagnosis.2.Strengthen the professional publicity for various chronic diseases to increase residents’knowledge about chronic disease, improve residents’ consciousness of chronic disease and its complications risk.3.Increase the proportion of reimbursement for the patients who choose the primary treatment, guide them to the reasonable medical treatment.
