Objective:Gastric serious threat to human life and health, chronic atrophic gastritis (CAG) is an important precancerous disease, often accompanied by gastric intestinal metaplasia, and dysplasia and other precancerous lesions.Intestinal metaplasia (Intestinal metaplasia, IM) is an important step in cancer links, therefore, how to effectively intervene intestinal metaplasia and reversal CAG gastric precancerous lesions CAG, blockits path of development, is the key to the current prevention of gastric cancer.But medicine is currently focused on the correct treatment of the cause, protect the gastric mucosa, etc.There is no more effective means.At present, Chinese medicine understanding of intestinal metaplasia is many, various andcomplex.Hepatogastric swelterand Gallbladderheat infringe onstomachis not uncommon in Clinical and literature.Chinese medicine is more effectivethrough a combination of diseases and syndromes, but the mechanism needs further exploration.This study is based on traditional Chinese medicine theory and research including clinical literature expert advice, by observing cases of the relationship between the clinical syndromes associated with intestinal metaplasia and clear-droptheory theoretical effect of the disease.Mainly studyed Hepatogastric swelter (Gallbladderheat infringe on stomach) type CAG with IM immunohistochemical index CDX2, MUC2, Ki-67 levels of distribution, explored the variation and trends of CDX2, MUC2, Ki-67 after the Gallbladder heat infringe on stomachtypeintestinal metaplasia treatment.The disease may be further elaborated in the pathogenesis and reverse inference theory of TCM clear-droptheory mechanism of the disease, rich theoretical basis and clinical diagnosis of the disease.Methods:This article includes theoretical study of disease and clinical research.The first part includes Chinese and Western reviewed and summarized the progress on understanding the disease.The first chapter summarizes the TCM Disease evolution and related diagnostic category, pathogenesis and classification of syndromes, and treatment research profiles. The second describes chronic atrophic gastritis with intestinal metaplasia modern medicine advances,including pathogenesis, prevention and treatment progress and so on.The second part of the clinical study, first,thestudy observed64 CAG with IMpatient’s clinical datafrom 2012.02-2014.12 Beijing University of Chinese Medicine, Dongzhimen Hospital. From the general information of four diagnostic information to start, mainly using descriptive analysis, such as differences in teststatistical methods,, exploratory study of immunohistochemistry indicators CDX2, MUC2 and Ki-67distribution characteristics of CAGwith IM. Second in Stomach qi stagnation,Hepatogastric swelter and Gallbladderheat infringe onstomach. Second observed Gallbladder heat infringe ons tomach symptoms,pathology, immunohistochemistry and other indicators of changebefore and afterdrop theory oral medicine treatment, to explore mechanism of the clear-lowering theory reversal intestinal metaplasia.Results:(1) This study suggests that male and female incidence ratio close and the higher prevalence of elderly.(2) CDX2 expression and intestinal metaplasia level is a positive linear correlation, that is, CDX2 expression upward trend with the aggravation of intestinal metaplasia in Stomach qi stagnation, Hepatogastric swelter and Gallbladderheat infringe onstomach. And so is MUC2 in Stomach qi stagnation and Gallbladderheat infringe onstomach.(3) Clear drop theory medicine treatment of biliary heat Fan Wei type IM, TCM Syndrome Evaluation:cured three cases, accounting for 13.64%, effective in 11 cases, accounting for 50.00 percent, effective in 6 cases, accounting for 27.27%and 2 ineffective, accounting 9.09%, the total efficiency of 90.91%.(4)Clear down theory treated group of bile heat Fan Wei type CAG with IM pathologic evaluation of the efficacy of before and after treatment:cured four cases, accounting for 18.18%, effective in 11 cases, accounting for 50 percent, effective in 3 cases, accounting for 13.64%,4 cases, accounting for 18.18%, the total efficiency= 81.82%.(5) There is difference of CDX2 integration after TCM treatment(P<0.05).Conclusion:(1) CDX2, MUC2 is a more reliable indicator of intestinal metaplasia, increased with the degree of intestinal metaplasia rises,but whether both increased in the three TCM type is not confirmed.(2) Clear drop theory hasreliable efficacy on CAG with IM.(3)Clear -drop theory may down CDX2, MUC2 expression of intestinal metaplasia and then improve or even reverse. But this observation, the effect of the hyperplasia intervention was not obvious.
