ObjectiveTaking spontaneously hypertensive rats as its experimental models, the aim of the subject is to observe the different effects of reinforcing and reducing methods by twirling and rotating the needle on the levels of renin and Angâ…¡ and the expression of VEGF in renal tissues of SHR, to investigate the plausible mechanism of reinforcing and reducing method on renal protection and to reveal the differences existing in reinforcing and reducing method. At the end, regarding this as a breakthrough point, to clarify the regulatory mechanism of twisting reinforcing and reducing method on blood pressure.MethodsSixty clean grade, male SHR (200-250g) of 12weels old were randomly divided into five groups,12 rats in each group. They were an acupuncture with reinforcing method group, an acupuncture with reducing method group, an even reinforcing-reducing method group, an acupuncture group and a model group. Twelve WKY rats served as blank group. Different acupuncture manipulations on "Taichong" point (LR3) were respectively applied on each acupuncture group by the same person in every day morning. The model group and blank group were just be grabbed like the acupuncture groups without acupuncture treatment. The treatment cycle was 28 days. Systolic and diastolic blood pressure were measured by tail cuff method one day before the start of the experiment and the 3th,6th, 10th,13th,17th,20th,24th,27th day after the experiment. At the end of the treatment, the pathologic changes of the renal tissue were observed by Hematoxylin-Eosin (HE) staining, the contents of renin and Angâ…¡ in blood plasma and renal tissue were measured with ELISA and VEGF expression in kidney was measured by immunohistochemistry assay.Results1 The renal pathological changes of each groupThe pathological changes in renal tissue were examined by optical microscopy. The results were as follows:The kidney tissues of WKY rats were basic normal; The model group, acupuncture group, reinforcing method group and reinforcing-reducing method group had different degrees mesangial cells and matrix hyperplasia, glomerular sclerosis, irregular thickening and atrophic membranes within the glomeruli, renal interstitial fibrosis and the smaller arteries in the kidney had become thickened. The model group had a little severer lesion compared with other groups. The vascular structure of reducing method group was basic normal, the glomerular and renal tubular structure were also normal.2 The changes of blood pressure of each group before and after treatmentThe systolic and diastolic pressure of SHR groups had no significant difference (p>0.05) before treatment. But they were significantly higher than blank group (P<0.01).28 days later, the systolic pressure of blank group showed a significant rise (p<0.05), but the diastolic pressure had no obvious change (p>0.05). The systolic and diastolic pressure of model group both increased (p<0.05). At the same time, obvious differences exist among reinforcing method group, reinforcing-reducing method group, acupuncture group and model group (p<0.05). There were no significant changes in the systolic and diastolic pressure of reinforcing method group, reinforcing-reducing method group and acupuncture group before and after treatment(p>0.05). The systolic and diastolic pressure of reducing method group decreased dramatically (p<0.05) and significant differences exist compared respectively with reinforcing method group, reinforcing-reducing method group and acupuncture group (p<0.05), at the same time it still showed a higher level than blank group (P<0.05).3 The content changes of renin and Angll in blood plasma of each groupThe contents of renin and Angll in blood plasma of model group were higher than that of blank group and reducing method group (P<0.01) and statistical difference exist when compared with reinforcing method group, reinforcing-reducing method group and acupuncture group (P<0.05). Reducing method group had a declined plasma renin and Angll levels when compared with reinforcing method group, reinforcing-reducing method group, acupuncture group and model group(P<0.01). No significant differences exist between reducing method group and blank group (P>0.05). There were no obvious difference exist between reinforcing method group, reinforcing-reducing method group and acupuncture group (P>0.05).4 The content changes of renin and Ang â…¡ in renal tissue of each groupThe contents of renin and Angll in renal tissue of model group were higher than that of blank group and reducing method group (P<0.01) and statistical difference exist when compared with reinforcing method group, reinforcing-reducing method group and acupuncture group (P<0.05). The renin and Angll levels in renal tissue of reducing method group decreased when compared with reinforcing method group, reinforcing-reducing method group, acupuncture group and model group(P<0.01). No significant differences exist between reducing method group and blank group (P>0.05).There were no obvious difference exist between reinforcing method group, reinforcing-reducing method group and acupuncture group (P>0.05).5 The VEGF expression of each groupThe the expression of VEGF in model group were higher than that of blank group and reducing method group (P<0.01) and statistical difference exist when compared with reinforcing method group, reinforcing-reducing method group and acupuncture group (P<0.05). VEGF expression in reducing method group was obviously decreased comparing with other acupuncture groups (P<0.01) and statistical difference also exist when compared with blank group (P<0.05).Conclusions1 Acupuncture with twirling reducing method can effectively reduce the blood pressure levels of the 12-week-aged SHRs, lower the levels of renin and Angâ…¡ in blood plasma and renal tissue and lessen VEGF expression in renal tissue;2 Acupuncture with twirling reducing method can effectively reduce the blood pressure levels and hypertensive kidney lesion.The plausible mechanisms were as follows:â‘  Twirling reducing method can effectively lower the levels of renin and Angâ…¡, which will consequently reduce the role of RAS in hypertensive kidney lesion.â‘¡Twirling reducing method can lessen the VEGF expression in renal tissue which will consequently reduce the kidney damage caused by VEGF;3 Great differences exist between twirling reducing method and twirling reinforcing method in this experiment.
