Aim:1. Investigate the possibility of using speckle tracking imaging technology forquantitatively analyzing right ventricular function early change of chronic obstructivepulmonary patient2. Use speckle tracking imaging for quantitatively evaluating the curative effect oflong-term oxygen therapy to right ventricular function of chronic obstructive pulmonarypatientMethod:Two Groups are select.Case group:40pulmonary function level I and chronicobstructive pulmonary patients whose two dimensional ECG are normal checked duringOct2011to March2012and who can insistently take low flow rate oxygen uptake(2L/Min)10hours per day for one year. Control group:40healthy volunteer without anyhistory of heart disease, chronic obstructive pulmonary and clinical symptoms. TheirECG,chest radiography and echocardiogram are normal.1.Measure and compare the RVEDV, RVESV,RVSV,RVEF,T1,T2,Tm,Tm%, Sbasfw,SRbasfw,Smidfw,SRmidfw,Sapifw and SRapifw between case group and control groupafter1day,3months,6months and one year oxygen uptake with colorful Dopplerinspection.2.Measure and compare the FEV1, FVC and the percentage of FEV1between casegroup and control group after1day,3months,6months and one year oxygen uptake withlung function inspectionResult:1day before the oxygen uptake, T1、T2、Tm、Tm%of COPD is dramatically lowerthan control group (P<0.05).1day before the oxygen uptake, Sbasfw、Smidfw、Sapifw ofCOPD has statistical meanings difference,compared with control group (P<0.05).1daybefore the oxygen uptake, SRbasfw、SRmidfw、SRapifw of COPD are dramatically lowerthan control group(P<0.05).T1,T2,Tm,Tm%are positively correlated with RVEF(P<0.01),rare0.777、0.613、0.831、0.849. Sbasfw、Smidfw、Sapifw、SRbasfw、SRmidfw、SRapifw are all negatively correlated with RVEF(p<0.01), rare-0.562、-0.557、-0.54、-0.529、-0.506、-0.443.2.Sbasfw、SRbasfw、Smidfw、SRmidfw are higher after3month oxygen uptake than1day oxygen uptake which has statistical meanings. The Sbasfw and SRbasfw obviouslyincrease with oxygen uptake time (P<0.01). Tm%increases after3months oxygen uptake.It is apparently different from data before oxygen uptake and continues increasing withoxygen uptake time (P<0.01). T1,T2and Tm increase after6month which has statisticaldifference with data before oxygen uptake and continues increasing with oxygen uptaketime. FEV1, the percentage of FEV1increase after1year oxygen uptake which hasstatistical difference.Conclusion:1.Patients with chronic obstructive pulmonary will have right ventricular systolicfunction attenuation at early stage.2.Long term oxygen treatment can significantly improve the right ventricular systolicfunction of chronic obstructive pulmonary patient. It has earlier improvement on rightventricular function than lung function.3.Speckle tracking imaging technology can sensitively and accurately estimate theright ventricular systolic function change condition of chronic obstructive pulmonarypatients before and after oxygen uptake.
