Acute myocardial infarction(AMI)has the risk of death and disability at the first onset.Exploring the regularity of AMI onset time is beneficial to take measures to prevent AMI and optimize treatment.This study analyzed the characteristics and potential mechanisms of the onset time of AMI patients from two aspects based on the Five Evolutive Phases and Six Climatic Factors and circadian rhythm gene respectively,aiming to provide more ideas and research evidence for clinical prevention and treatment of AMI.Objective1 Exploring the five evolutive phases and six climatic factors rules of birth date and onset date of AMI patients,to supplement more comprehensive clinical research evidence.2 Exploring the circadian rhythm of the onset time of AMI patients,and the expression level of circadian rhythm genes in peripheral blood mononuclear cell(PBMC),to supplement more clinical research evidence for the mechanism of the regularity of AMI onset time.Methods1 Extracted the AMI patient data from January 2011 to December 2020 in database.Statistical analysis was adopted to explore the distribution of five evolutive phases and six climatic factors of the birth date and onset date of the patients.2 Extracted the AMI patient data from January 2011 to December 2020 in database.Statistical analysis was adopted to explore the circadian rhythm of onset time,and the relationship between gender,age and other cardiovascular disease risk factors and the onset time.At the same time,AMI patients and healthy people were included,all subjects took peripheral venous blood from elbow vein at 05:00,11:00,17:00 and 23:00,to detect the expression level of melatonin in peripheral venous plasma,and the oscillation level of biological clock gene after extracting PBMC from venous blood.Results1 A total of medical record data of 1298 AMI patients were included for statistical analysis.There were 849 males with an average age of 64.58±13.69 years,and 449 females with an average age of 74.54±11.59 years.(1)The attributes of onset date of the most AMI patients were as follows,the year which evolutive phase was hypoactivity of yun qi of wood,the host evolutive phase which was Taishang,the gust evolutive phase which was Taizhi,the Sitian-Zaiquan which was Taiyang Hanshui-Taiyin Shitu(P<0.05).While the attributes of onset date of the least AMI patients were as follows,the year which evolutive phase was hypoactivity of yun qi of water,the host evolutive phase which was Taigong,the gust evolutive phase which was Taiyu,the SitianZaiquan which was Shaoyang Xianghuo-Jueyin Fengmu(P<0.05).(2)The attributes of birth date of the most AMI patients were as follows,the host evolutive phase which was Shaoshang,the host climatic qi which was Yangming Zaojin,the gust climatic qi which was Shaoyin Junhuo(P<0.05).While the attributes of birth date of the least AMI patients were as follows,the host evolutive phase which was Taizhi,the host climatic qi which was Shaoyin Junhuo,the gust climatic qi which was Taiyin Shitu(P<0.05).2 A total of 691 patients with AMI were included for statistical analysis.(1)For the overall AMI patients,the peak of onset time period was 02:00 to 07:59(P<0.05).For the AMI patients with male subgroup,no smoking history subgroup,no drinking history subgroup,hypertension subgroup,no hyperlipidemia subgroup,no obesity or overweight subgroup,and no obvious inducement subgroup,the peak of onset time period was 02:00 to 07:59(P<0.05).There were no obvious circadian rhythm the onset time of AMI patients with female subgroup,the young subgroup,drinking history subgroup,without hypertension subgroup,hyperlipidemia subgroup,and obvious inducements subgroup(P>0.05).For the AMI patients with the middle-aged subgroup,the old subgroup,smoking history subgroup,the peak of onset time period was 02:00 to 09:59(P<0.05).The peak of onset time period of obesity or overweight subgroup of AMI patients was 22:00 to 03:59 the next day(P<0.05).The peak of onset time period of AMI patients with diabetes was 00:00 to 07:59(P<0.05).(2)A total of 30 subjects were included,including 15 in the AMI group and 15 in the control group.Compared with the control group,the expression levels of melatonin in plasma,and the expression levels of CRY1 and CRY2 in PBMC significantly decreased at 5:00,11:00,17:00 and 23:00(P<0.01).While the expression levels of PER1,PER2 and PER3 in PBMC significantly decreased at 5:00,17:00 and 23:00(P<0.05).Conclusion1 There were differences in the distribution of onset date and birth date of AMI patients based on the different five evolutive phases and six climatic factors.2 The onset time of AMI patients has a circadian rhythm,and the circadian rhythm changes with different risk factors.3 The plasma melatonin level of AMI patients was lower than that of normal people,but the circadian rhythm still existed.The expression level of circadian rhythm genes(PER 1/2/3,CRY1/2)in PBMC of AMI patients were decreased,with the circadian rhythm disappeared.More studies are needed in the future to support the conclusion.
