| Background:In recent decades,most countries in the world have reported on the incidence and mortality of lung cancer.According to these reports,we have found that with the increase of time,the incidence and mortality of lung cancer also increase.The cause of lung cancer has not yet been clarified.As a developing country,with the rapid development of industrialization and urbanization,there are more and more environmental pollution and a large number of smokers,which induce the gradually increased incidence and mortality of lung cancer in China in recent years.The main risk factor affecting the prognosis of lung cancer is lymphatic metastasis.Many studies have shown that chemokines and their receptors play important roles in the process of lymph node metastasis.The trending factor receptors,including CX-Chemokine Receptor(CXCR),CXCR4 and CCR7,in the CCR family are involved in many malignancies.They are expressed in the tumor cells.In recent years,through in-depth clinical researches on tumor microenvironment and tumor immune escape mechanisms,we have discovered a group of important negative costimulatory molecules,including T cell immunoglobulin domain and mucin domain-3(TIM-3),cytotoxic T-lymphocyte-associated antigen 4(CTLA-4)and related molecules of the negative B7 family,such as the negative B7 family molecules CTLA-4 and TIM3.They not only have a mediating effect on immune regulation,but also reflect tumor tissues or infiltrating immune cells with specific expression.Studies have pointed out that they participate in the immune escape of tumors,and have obvious connections with the generation and development of tumor cells,and play important roles in the composition of tumor microenvironment.Immune regulation is an momentous part of the tumor microenvironment.So it is of great clinical value to study the expression levels of CXCR4 and CCR7 and negative costimulatory molecules TIM-3 in the tumor tissues of patients with non-small cell lung cancer.Objective:The effect of immune system function in patients with non-small cell lung cancer is significantly related to the severity of cancer.The purpose of this study was to explore the expression levels of chemokine receptor CXCR4 and CCR7 and negative costimulatory molecules TIM-3 on patients with non-small cell lung cancer.Methods:A total of 100 patients with non-small cell lung cancer who went to the Department of Thoracic Surgery of the Second Affiliated Hospital of Zhengzhou University from January 2017 to December 2017 were included in our study.Immunohistochemistry was used to detect the expression of CXCR4,CCR7 and TIM-3 in NSCLC patients.The expression of CXCR4 and CCR7 in different tissues was compared.The relationship between the expression of CXCR4,CCR7 and clinical parameters and the prognosis of patients with NSCLC were analyzed.To analyze the relationship between CCR7 and CXCR4 protein expression in non-small cell lung cancer patients.Objective to investigate the expression of Tim-3 in T lymphocytes of non-small cell carcinoma(NSCLC),normal lung tissue and peripheral blood.Objective to analyze the relationship between Tim-3 expression and clinical parameters in patients with non-small cell lung cancer.SPSS22.0 statistical software was used for statistical analysis.χ2 test,independent sample t test,One-way ANOVA,Mann-Whitney U test,Log-Rank test were used for difference comparision,and the test level was a=0.05.Results:1.In the tumor tissues of patients with non-small cell lung cancer,the protein expression rate of CXCR4 was high to 46%.The protein expression rate of CXCR4in the distal lung tissue was high to 7.3%(P<0.05);2.The relationship between the expression of CXCR4 and the clinical indicators of patients with non-small cell lung cancer.The CXCR4 protein expression of patients with non-small cell lung cancer was not correlated with gender,age,smoking history,degree of differentiation,clinical stage,pathological type(P>0.05).However,the CXCR4 expression of patients with non-small cell lung cancer was related to lymph node metastasis and tumor stage(P<0.05);3.Comparing the correlation between the CXCR4 expression level and the prognosis of patients with non-small cell lung cancer:the prognosis of patients in the CXCR4 high expression group was significantly poorer than that of patients in the CXCR4 low expression group(P<0.05);4.The expression rate of TIM-3 in CD4~+T lymphocytes in non-small cell lung cancer tumor tissues(32%±9.98%)was significantly higher than the expression rate in CD4~+T lymphocytes in distant normal lung tissues(17%±10.01%)。The expression rate of TIM-3 in peripheral blood CD4~+T lymphocytes of patients with non-small cell lung cancer was lower,which was(2.6%±0.87%),and the difference was statistically significant(P<0.05);5.The expression rate of TIM-3 in CD8+T lymphocytes in non-small cell lung cancer tumor tissues was(33%±10.96%).In the distal lung tissue,the expression rate of TIM-3 in CD8~+T lymphocytes decreased to(13%±8.26%).The expression rate of TIM-3 in peripheral blood CD8~+T lymphocytes of patients with non-small cell lung cancer was low(3.6%±1.43%),and the difference was statistically significant(P<0.05);6.The expression levels of TIM-3 in patients with non-small cell lung cancer had no significant differences with gender,age,tumor size and tissue type(P>0.05),however,there is a marked correlation between tumor size,lymph node metastasis and tumor node metastasis(TNM)staging in patients with non-small cell lung cancer(P<0.05);7.The CCR7 protein expression rates in tumor tissue specimens were higher than those of normal lung tissues(P<0.05);8.The relationship between the expression of CCR7 and the clinical indicators of patients with non-small cell lung cancer.The CCR7 protein expression of patients with non-small cell lung cancer was not correlated with gender,age,smoking history,degree of differentiation,pathological type(P>0.05).However,the CCR7 expression of patients with non-small cell lung cancer was related to tumor size,lymph node metastasis and tumor stage(P<0.05);9.The survival status of patients was analyzed according to the level of CCR7protein expression,and the results showed that the survival rate of patients in the CCR7 protein high expression group was significantly lower than that of the CCR7protein low expression group(P<0.05);10.Spearman rank correlation analysis was used to assess the association of the protein expression levels of CCR7 and CXCR4 in tumor tissues of patients with non-small cell lung cancer:The results showed that the expression levels of CCR7and CXCR4 were significantly correlated(P=0.001,correlation coefficient=0.265).Conclusion:1.The expression levels of chemokine receptors CXCR4 and CCR7 in tumor tissues of patients with non-small cell lung cancer were significantly higher than those in normal lung tissues.2.The abnormal expression of chemokine receptors CXCR4 and CCR7 is related to tumor size,lymph node metastasis,tumor stage and prognosis in patients with non-small cell lung cancer.3.The TIM-3 expression level of CD4+T and CD8+T lymphocytes in tumor tissues of patients with non-small cell carcinoma is higher than that in normal lung tissue and peripheral blood.4.The expression of TIM-3 in tumor tissues of patients with non-small cell carcinoma is related to tumor size,lymph node metastasis,and tumor stage. |
