Background:At present,the decline of male semen quality in some areas of China is becoming more and more serious,which has attracted the attention of all sectors of society.The number of patients coming to Wenzhou for infertility is increasing year by year.It is urgent to understand the distribution and causes of male infertility in this area.Evidence shows that urogenital tract infection can lead to male infertility,and lipopolysaccharide(LPS)can mediate urogenital tract inflammation.The team’s previous research found that L-carnitine(LC),as an antioxidant,can play a protective role in LPS induced acute reproductive system injury in male mice through mechanisms such as antioxidant stress,regulating fatty acid metabolism and reducing inflammatory response.However,whether LC has a protective effect on the chronic injury caused by it and its mechanism are not clear.Objective:To study the change trend of semen quality of infertile men in Wenzhou in recent years;To observe the protective effect and mechanism of LC in LPS induced chronic testicular injury in male mice.Methods:1.Clinical research:338905 infertile patients who came to the urology and reproductive andrology Department of our hospital from January 2008 to November 2016 were collected,their age,occupation and semen routine results were recorded,and the change trend of semen quality of male infertile patients in Wenzhou was analyzed.2.Experimental study:Thirty C57BL/6jgpt male mice were randomly divided into three groups with 10 mice in each group.The model was made by intraperitoneal injection.The administration groups were as follows:group A:0.9%sodium chloride injection;Group B:LPS solution modeling;Group C:LPS solution modeling+LC solution intervention.After 4 weeks of modeling and administration,eyeball blood was collected and dissected after anesthesia.It is used for semen quality evaluation,histomorphological observation,inflammatory factors,oxidative antioxidant stress indicators and fatty acid metabolic pathway molecular detection.Results:1.Semen parameters of 24.9%of infertile patients in Wenzhou were normal.The semen quality of workers group and 40 year old group was significantly lower than that of other occupational groups and age groups.Low semen volume,asthenospermia and teratozoospermia accounted for 8.4%,50.5%and 54.1%respectively.The annual percentage of grade a sperm,Grade A+B sperm and normal sperm decreased linearly;The proportion of patients with asthenospermia and multiple semen parameter abnormality syndrome increased linearly,while for patients with oligozoospermia,the proportion decreased linearly.2.LC can improve sperm motility and sperm concentration in LPS induced chronic testicular injury in male mice;Through morphological observation,the structure and cell morphology of testis were damaged after 4 weeks of LC intervention,but the degree of damage was significantly lower than that of non intervention mice.LC improves PPAR-γ Reduce the level of inflammatory factors;100mg/kg LC has obvious antioxidant effect on oxidative stress injury of testis in male mice induced by LPS;LC can increase the expression of OCTN2 and CPT1 A,the key molecules of fatty acid metabolism;In the chronic testicular injury of male mice induced by LPS,the expression of FABP9 protein in testicular tissue increased.LC intervention can reduce its expression,which proves that the degree of inflammation is closely related to FABP9.Conclusion:1.The semen quality of infertile men in Wenzhou decreases year by year,and is often accompanied by a history of urogenital infection.Therefore,the research on urogenital infection and male semen quality should attract the attention of relevant people.2.LC can play a major role in LPS induced chronic testicular injury by antioxidation,reducing inflammatory response and regulating fatty acid metabolism.Its antioxidant effect is better than acute injury,which provides theoretical support and experimental basis for the clinical application of LC in the treatment of male reproductive tract infection.
