Objective:This study is a combination of traditional Chinese medicine acupuncture theory and assisted reproductive technology(ART).The retrospective and prospective studies were used to explore the synergistic effect of kidney tonifying and liver regulating acupuncture on the pregnancy outcomes of patients with in vitro fertilization-embryo transfer(IVF-ET)in eastern Canada.A retrospective study aimed to investigate the effectiveness of acupuncture on clinical pregnancy outcomes of women undergoing in vitro fertilization-embryo transfer fresh cycles with elective single embryo transfer(eSET);a prospective study aimed to observe the effects of acupuncture on clinical pregnancy outcomes in advance aged infertile DOR women over 35 years old who underwent the minimal stimulation protocol(MSP).Method 1:A retrospective,self-controlled study,1292 women undergoing IVF-ET with their own eggs,fresh cycle,elective single embryo transfer(eSET).At least one history of unsuccessful IVF cycle,ages between 25 and 42 years old,conducted at five IVF clinics in Montreal between October 1,2010 and October 30,2015,with 10 months of pregnancy follow-up until August 31,2016.1292 IVF patients received two sessions of acupuncture on the same day of embryo transfer:30 minutes before and after fresh embryo transfer.Pregnancy care acupuncture treatment was given untill the week of labor after the BT positive.Outcomes measured were biochemical,clinical pregnancy and live birth rates.Results:The influence of acupuncture and moxibustion on the pregnancy outcome of IVF-ET fresh cycle single embryo transplant patients:(1)the average clinical pregnancy rate(CPR)and the live pregnancy rate(LBR)were 67.41%and 60.76%respectively in the two acupuncture and moxibustion groups on the transplantation day;(2)the clinical pregnancy rate by age stratification was<35 years(75.37%),35-40 years(54.60%)and>40 years(42.47%),which were statistically different from<35 years(P<0.05))(3)the live birth rates by age stratification were<35 years old(67.52%),35-40 years old(49.57%),and>40 years old(26.55%).Compared with<35 years old,there were statistically significant differences(P<0.05);(4)compared with the average clinical pregnancy rate(CPR)in Canada in 2016,the live birth rates were<35 years old(75.57%vs 40%),35-40 years old(54.66%vs 35%),and>40 years old(42.47%vs 29%).Compared with each age stratification,there was statistical difference(P<0.05).Method 2:A prospective self-controlled study was conducted to investigate the clinical outcomes of 103 advance aged infertile women over 35 years old who failed to get pregnant in the first IVF-ET cycles with long protocol.They underwent the minimal stimulation protocol combined acupuncture in the second or third cycle between June 1,2017 and June 1,2019.A follow up of pregnancy tracking was done untill September 16,2019.A total of 103 IVF-ET cycles with minimal stimulation protocol were associated with eight sessions of acupuncture.After acupuncture intervention,the primary outcomes included good-quality embryo rate(GER),clinical pregnancy rate(CPR)and live birth rate(LBR)of Embryos transfer(ET).The secondary outcomes were endometrial lining thickness and types,the number of oocytes retrieval,available embryos,fertilization rate and ongoing pregnancy rate(OPR).Results:The results showed that the minimal stimulation protocol associated with acupuncture can provide better clinic outcomes for the diminished ovarian reserve(DOR)patients who failed to get pregnant with long protocol in IVF-ET.The oocyte utilization rate and good-quality embryo rate were significantly higher after acupuncture.The clinical pregnancy rate,live birth rate and ongoing pregnancy rates are 65%,34%and 15%.Group divided by age,CPR of acupuncture group compares with Montreal Fertility centre(MFC)’s CPR:35-37 58%VS 44%、38-40 42%VS 35%,P<0.05。...
