Knee osteoarthritis(KOA),as one of the common clinical arthritis,is the main cause of inconvenience in elderly patients,and the proportion of physical disabilities is very high,which seriously affects the quality of life and mental health of patients.Modern medicine has not yet clarified the cause of the disease,and the specific pathogenesis is also unclear.Once it occurs,the course of the disease is almost irreversible.At present,there is no specific drug that can significantly reverse the progression of knee osteoarthritis,and there is no clinically clear cure for the disease.Traditional Chinese medicine has a long history in the treatment of knee osteoarthritis.In recent years,a large number of studies have confirmed that the Jin’s 3-Knee Points therapy and YiGuan therapy(under the guidance of the meridian theory,combined cupping,exercise,and massage)are used in the KOA is safe and effective,has a low recurrence rate and few side effects,so it is of great clinical research value.Object iveNowadays society is operating at a high speed,and patients seek to obtain better treatment results in the shortest possible time.Quite a few patients are not satisfied with the efficacy of monotherapy.Therefore,this study adopted a randomized controlled trial to observe the combination of Jin’s 3-Knee Points therapy and YiGuan therapy.Whether the clinical effectiveness and timeliness of the therapy in the treatment of knee osteoarthritis is better than Jin’s 3-Knee Points therapy or YiGuan therapy alone,so as to make a preliminary study on the clinical efficacy and safety of Jin’s 3-Knee Points therapy combined with YiGuan therapy for the treatment of the disease Analyze,clarify the preliminary treatment mechanism of Jin’s 3-Knee Points combined with YiGuan in the treatment of KOA,and further explore the clinical value and significance of "Jin’s 3-Knee Points combined with YiGuan" in the treatment of knee joints.In addition,through research to provide an optimized and quick-acting treatment plan for the clinical treatment of this disease,and provide a certain reference basis for future clinical promotion.methodAll subjects in this study are from patients with KOA who were in the outpatient clinic of Jingcheng Chinese Medicine Specialist Clinic in Mong Kok District,Hong Kong from January 2019 to January 2020.A total of 90 patients were enrolled in this study,and a random number table was generated through the STATA 11.0 software.The 90 patients were entered into the corresponding group in the random number table according to the order of treatment,and they were randomly divided into the observation group(Jin’s 3-Knee Points therapy combined with YiGuan therapy).Jin’s 3-Knee Points group(Jin’s 3-Knee Points therapy),and YiGuan group(YiGuan therapy),each with 30 cases The course of treatment is 1 treatment every other day,and 5 times is a course of treatment,a total of 2 courses of treatment,and 3 days rest between the two courses.Insist on functional exercise every day,and follow-up one month after the end of 2 courses.The main indicators for the evaluation of the curative effect of this study are the knee osteoarthritis index scale(WOMAC)score evaluation,and the secondary indicators include the visual analogue scale(VAS)score,the knee osteoarthritis treatment effect judgment standard scale(JOA)score,Depression Short Scale(CDS-15)score.The three groups of subjects were scored four times before treatment,after treatment for 1 course,after treatment for 2 courses,and 1 month after the end of treatment.The differences in their own scores before and after treatment and the differences in scores between the three groups were compared,and three groups were collected.The clinical data of the group were analyzed using STATA 11.0 software,so as to conduct preliminary research and analysis on the clinical efficacy and safety of Jin’s 3-Knee Points combined with YiGuan therapy for knee osteoarthritis.Results:(1)Baseline data:A total of 90 cases were included in this study,and they were randomly divided into observation group,Jin’s 3-Knee Points group,and YiGuan group,each group has 30 cases.In the course of research and treatment,no cases were removed,and 90 cases were finally completed.The completion of the three groups is basically the same.Before treatment,the three groups of subjects were assessed by KL grade,visual analogue scale(VAS)score,knee osteoarthritis index(WOMAC)score,and knee osteoarthritis in the three groups of subjects in terms of gender,age,course of disease,and location of onset.There was no significant difference in the scores of the Treatment Outcome Judgment Standard Scale(JOA)and the Short Depression Scale(CDS-15)(P>0.05).The baselines were consistent,and the groups were comparable.(2)Visual analogue scoring(VAS)score:Comparison between groups:after 1 course of treatment,there was no difference in VAS score between the Jin’s 3-Knee Points group and the YiGuan group(P>0.05).The VAS score of the observation group was lower than that of the YiGuan group(P<0.05),no difference from the three-needle knee acupuncture group(P>0.05).However,after 2 courses of treatment and 1 month after the end of treatment,there was no difference in VAS scores between the Jin’s 3-Knee Points group and the YiGuan group(P>0.05),and the VAS scores of the observation group were significantly lower than those of the Jin’s 3-Knee Points and YiGuan group(P<0.01).It shows that the effect of Jin’s 3-Knee Points and YiGuan therapy in improving knee joint pain symptoms is significantly better than that of simple Jin’s 3-Knee Points or YiGuan therapy;Jin’s 3-Knee Points or YiGuan therapy is equivalent in improving knee pain symptoms.Intra-group comparison:The VAS scores of the three groups of patients were significantly reduced as the treatment progressed(P<0.01),indicating that the three treatment options improved the knee joint pain after treatment for 1 course,2 courses,and 1 month after treatment.The symptoms have obvious effects.There was no difference between the VAS scores of the three groups of patients in 1 month after treatment and the VAS scores after 2 courses of treatment(P>0.05),indicating that the above three treatment options can alleviate the recurrence of knee pain symptoms.(3)Knee Osteoarthritis Index(WOMAC)score:① WOMAC pain score:comparison between groups:after 1 course of treatment,there was no difference in WOMAC pain score between the Jin’s 3-Knee Points group and the YiGuan group(P>0.05).The WOMAC pain score of the observation group was lower than that of the YiGuan group(P<0.01),and compared with the knee There was no difference in the three-needle group(P>0.05).But after 2 courses of treatment and 1 month after the end of treatment,there was no difference in WOMAC score between the Jin’s 3-Knee Points group and the YiGuan group(P>0.05),and the WOMAC pain scores of the observation group were lower than those of the Jin’s 3-Knee Points and YiGuan group(P<0.05 and P<0.01).It shows that the effect of Jin’s 3-Knee Points and YiGuan therapy in improving knee joint pain symptoms is significantly better than that of simple Jin’s 3-Knee Points or YiGuan therapy;Jin’s 3-Knee Points or YiGuan therapy is equivalent in improving knee pain symptoms.Intra-group comparison:The WOMAC pain scores of the three groups of patients were significantly reduced as the treatment progressed(P<0.01),indicating that the three treatment options symptoms in patients.②WOMAC stiffness score:comparison between groups:after 1 course of treatment,there is no difference in WOMAC stiffness score between the three groups(P>0.05),but after 2 courses of treatment and 1 month after the end of treatment,the Jin’s 3-Knee Points group and the YiGuan group There was no difference in WOMAC stiffness scores(P>0.05).The WOMAC stiffness scores of the observation group were lower than those of the Jin’s 3-Knee Points group and the YiGuan group(P<0.01).It shows that the effect of Jin’s 3-Knee Points and YiGuan therapy in improving the symptoms of knee joint stiffness is significantly better than that of simple Jin’s 3-Knee Points or YiGuan therapy;while Jin’s 3-Knee Points or YiGuan therapy is equivalent in improving the symptoms of knee joint stiffness.Intra-group comparison:The WOMAC stiffness scores of the three groups of patients were significantly reduced as the treatment progressed(P<0.01),indicating that the three treatment options improved the knee stiffness of patients after 1 course of treatment,2 courses of treatment,and 1 month after treatment.The symptoms have obvious effects.There was no difference between the WOMAC stiffness scores of the three groups of patients in 1 month after treatment and the WOMAC scores after 2 courses of treatment(P>0.05).It shows that the above three treatments can alleviate the recurrence of knee stiffness in patients.③WOMAC function score:comparison between groups:after 1 course of treatment,2 courses of treatment and 1 month after the end of treatment,there was no difference in WOMAC function score between the Jin’s 3-Knee Points group and the YiGuan group(P>0.05),the observation group’s WOMAC function The scores were lower than those of the Jin’s 3-Knee Points group and the YiGuan group(P<0.05 and P<0.01).It shows that the effect of Jin’s 3-Knee Points and YiGuan therapy in improving knee joint function is significantly better than that of Jin’s 3-Knee Points or YiGuan therapy alone;while Jin’s 3-Knee Points or YiGuan therapy is equivalent in improving knee joint function.Intra-group comparison:The WOMAC function scores of the three groups of patients were significantly reduced as the treatment progressed(P<0.01),indicating that the three treatment options improved the knee joints of patients after 1 course of treatment,2 courses of treatment,and 1 month after treatment.The function has obvious curative effect.There was no difference between the WOMAC function scores of the three groups of patients in 1 month after treatment and the WOMAC function scores after 2 courses of treatment(P>0.05).It shows that the above three treatments can alleviate the recurrence of knee joint function in patients.④WOMAC total score:comparison between groups:after 1 course of treatment,2 courses of treatment and 1 month after the end of treatment,there was no difference in WOMAC total score between the Jin’s 3-Knee Points group and the significantly reduced as the treatment progressed(P<0.01),indicating that the three treatment options improved the knee bones of patients after 1 course of treatment,2 courses of treatment,and 1 month after treatment.Arthritis symptoms(including pain,stiffness,function)have obvious effects.There was no difference between the total WOMAC scores of the three groups of patients in 1 month after treatment and the WOMAC total scores after 2 courses of treatment(P>0.05).It shows that the above three treatments can alleviate the recurrence of knee osteoarthritis symptoms(including pain,stiffness,function).(4)Knee Osteoarthritis Treatment Effect Judgment Standard Scale(JOA)score:Comparison between groups:JOA score of Jin’s 3-Knee Points group and YiGuan group after 1 course of treatment,after 2 courses of treatment and 1 month after the end of treatment There was no difference(P>0.05).The JOA scores of the observation group were lower than those of the Jin’s 3-Knee Points group and the YiGuan group(P<0.01).It shows that Jin’s 3-Knee Points combined with YiGuan therapy is significantly better than Jin’s 3-Knee Points or YiGuan therapy in improving knee osteoarthritis symptoms(including pain,stiffness,and swelling);while Jin’s 3-Knee Points or YiGuan therapy is improving patients The effects of knee osteoarthritis symptoms(including pain,stiffness,and swelling)are poor.Intra-group comparison:The JOA scores of the three groups of patients were significantly increased as the treatment progressed(P<0.01).indicating that the three treatment options improved the knee bones of patients after 1 course of treatment,2 courses of treatment,and 1 month after treatment.Arthritis symptoms(including pain,stiffness,swelling)have obvious effects.The JOA scores of the three groups of patients in 1 month after the treatment were not different from those after 2 courses of treatment(P>0.05).It shows that the above three treatments can alleviate the recurrence of knee osteoarthritis symptoms(including pain,stiffness,swelling).(5)Short Depression Scale(CDS-15)score:Comparison between groups After one course of treatment,there was no significant difference in CDS-15 score between the three groups(P>0.05).However,after 2 courses of treatment and 1 month after the treatment,there was no difference in CDS-15 scores between the Jin’s 3-Knee Points group and the YiGuan group(P>0.05),and the CDS-15 scores of the observation group were lower than those of the Jin’s 3-Knee Points group and the YiGuan group.Group(P<0.01).It shows that Jin’s 3-Knee Points combined with YiGuan therapy is significantly better than simple Jin’s 3-Knee Points or YiGuan therapy in improving patients’ depression;while Jin’s 3-Knee Points or YiGuan therapy has poor efficacy in improving patients’depression.Intra-group comparison:The CDS-15 scores of the three groups of patients were significantly reduced as the treatment progressed(P<0.01),indicating that the three treatment options were respectively after 1 course of treatment,after 2 courses of treatment,and 1 month after treatment.Emotions have obvious effects.There was no difference between the CDS-15 scores of the three groups of patients in 1 month after the treatment and the CDS-15 scores after 2 courses of treatment(P>0.05).It shows that the above three treatment options can alleviate the recurrence of depression in patients.(6)Evaluation of clinical efficacy:After treatment,the total effective rate of the Jin’s 3-Knee Points group was 83%,5 cases were ineffective(17%),17 cases were effective(57%),6 cases were markedly effective(20%),and 2 cases were cured(6%);the total effective rate of the YiGuan group is 83%,5 cases are ineffective(17%),21 cases are effective(70%),3 cases are markedly effective(10%),and 1 case is cured(3%);the total effective rate of the observation group,it was 97%,ineffective in 1 case(3%),effective in 5 cases(17%),markedly effective in 19 cases(63%),and cured in 5 cases(17%).The difference in efficacy between the groups was statistically significant(P<0.05).Among them,there was no difference in efficacy between the three-needle knee and canning group(P>0.05).The efficacy of the observation group was better than that of the three-needle knee and canning group.(7)Safety assessment:There were only 4 people during the study.During the treatment,they developed mild nausea,hematoma,or local skin pain.After symptomatic treatment,the symptoms were quickly relieved without any aggravation.There were no other serious accidents and adverse reactions in the three groups,suggesting that the three treatment methods are relatively safe.Conclusion(1)Jin’s 3-Knee Points combined with YiGuan therapy is more effective than Jin’s 3-Knee Points therapy or YiGuan therapy.It can effectively relieve knee joint pain,reduce knee joint stiffness and swelling,and improve knee joint functional activities.Reduce the patient’s depression index,thereby improving the patient’s quality of life,and helping the patient’s mental health development.(2)The long-term maintenance effect of Jin’s 3-Knee Points therapy combined with YiGuan therapy in treating knee osteoarthritis is more significant than that of simple knee Jin’s 3-Knee Points therapy or YiGuan therapy,and can more effectively reduce the recurrence of the disease.(3)Jin’s 3-Knee Points combined with YiGuan therapy is safe for the treatment of KOA.There was no aggravation and obvious adverse reactions in the three groups.It can be seen that the three therapies are safe and effective. |
