| Objective:To investigate the relationship between serum mi RNA-217 and acute pancreatitis(AP)and its severity.Methods:(1)Selected 230 patients with AP admitted to baise people’s hospital on January 1,2018 and December 31,2019,all of them were newly diagnosed.According to the classification criteria,they were divided into clinically mild acute pancreatitis(MAP),Moderately severe acute pancreatitis(MSAP),and most clinically severe acute pancreatitis(SAP).The number of cases in MSAP patients was too small to be included in this study.The number of selected patients was 60 in MAP group and 60 in SAP group.Another 60 patients who came to the hospital for physical examination were selected as the control group.(2)General data of the patients including:gender,age,etiology(biliary,hyperlipidemic,alcoholic,and others),and the duration of hospitalization were collected.(3)Venous blood of each patient was collected within 24 hours after attack from AP,and procalcitonin(PCT)were examined,and the serum level of mi RNA-217 was measured by real-time quantitative PCR.(4)Early phase:Modified Marshall scoring system,APACHE II score(Acute Physiology and Chronic Health Evaluation II score),MCTSI score(A Modified CT Severity Index score),and BISAP score(Bedside Index for Severity in Acute Pancreatitis)were evaluated within 24-48 hours(early phase)after admission.(5)The expressions of serum PCT,and mi RNA-217 of the patients were measured 24hours before discharge(late phase).In addition,60 healthy people were taken as control group,and their venous blood were taken to detect the expressions of serum PCT,and mi RNA-217.Statistics:(1)The relation between serum mi RNA-217 and AP were analyzed.(2)The expressions of serum mi RNA-217in the early and late phases of AP were analyzed.(3)The relation among serum mi RNA-217 and modified Marshall scoring system,APACHE II score,MCTSI score,BISAP score,serum PCT were analyzed.(4)The sensitivity,specificity,positive predict value(PPV),negative predict value(NPV),accuracy and positive likelihood ratio of mi RNA-217 were analyzed for the prediction of SAP.Results:(1)The expression of mi RNA-217 was increased in AP patients.(2)The expression of mi RNA-217 in SAP group was significantly increased than that in MAP group(P<0.001).(3)The expression of mi RNA-217 in late period was significantly lower than that in acute period(P<0.001).(4)The serum level of mi RNA-217 and modified Marshall scoring system,APACHE II score,MCTSI score,BISAP score,and serum PCT were positively correlated.(5)Serum mi RNA-217 could be used as an index to predict the severity of AP.When the concentration of mi RNA-217=2.988×10~4 copies/m L,the sensitivity of SAP prediction was 100%,the specificity was 96.7%,the PPV was 94.0%,the NPV was 99.9%,the accuracy was 98.3%,and the positive likelihood ratio was 30.Conclusions:(1)Serum mi RNA-217 in acute pancreatitis patient is increased.(2)The serum mi RNA-217 in SAP is higher than MAP;(3)mi RNA-217 is positively correlated with modified Marshall scoring system,APACHE II score,MCTSI score,BISAP score,and serum PCT level,which could be used as a serological marker to predict the severity of acute pancreatitis in clinic. |
