The Law Study On Pathogenesis-therapy And The Systematic Review About Nourishing Kidney Drugs On Ankylosing Spondylitis

Objective:To analyse the law of pathogenesis and therapy of ankylosing spondylitis (AS) through researching the characteristics of Pathogenesis and drug therapy on literature study; to systematically review the efficacy and safety of nourishing kidney drugs against the basic mechanism for AS, so as to provide reference for clinical treating AS by Traditional Chinese Medicine.Method:Search the random and semi-random treatment studies of Traditional Chinese medicine in the past 20 years; establish the databases of the law on pathogenesis and therapy and the systematic review; explore the pathogenesis characteristics of AS by frequency analysis and factor analysis; explore the drug characteristics of the disease by frequency analysis; research the efficacy and safety of nourishing kidney drugs by Meta-analysis.Results:115 documents are bringed in the pathogenesis and therapy law study. In the total literature, the pathogenesis of kidney deficiency accounted for 82.61%, wet evil 79.13%, blood stasis 75.65%, the rest gradually as pathogenic wind, cold pathogen, qi deficiency, heat evil, liver deficiency, Phlegm, blood deficiency; the pathogenesis forms five factors:Fl including cold pathogen and heat evil, F2 including pathogenic wind, wet evil and meridian barrier, F3 including kidney, liver and blood deficiency, F4 including blood deficiency, Phlegm, meridian barrier, F5 including qi and blood deficiency. Characteristics of drug therapy includes of nourishing kidney, dispelling rheumatism, dredging blood circulation and relieving pain mainly.The main nourishing kidney drugs include Woodwardia, Eucommia, Dipsacus etc. The dispelling rheumatism drugs include Doubleteeth Pubescent Angelica, Clematis, Aconitum, etc. The dredging blood circulation drugs include Chinese angelica, red peony root, Cyathula, etc. The elieving pain drugs include Asaru, Scorpion, etc. The antipyretic drugs includ Cork, Anemarrhenae, Silver Flower, dispelling dampness drugs Coix Seed, Tuckahoe, Smilax glabra, etc. The nourishing spleen-qi include Radix, Codonopsis, etc. The eliminating phlegm drugs include Brassica alba, Southern Star, tangerine peel, etc. The Systematic review subsumes 33 studies, by Meta analysis, the total effective rate of nourishing kidney drugs group was significantly better than the control group, and the safety is significantly higher.Conclusion:The Basic pathogenesis of AS are kidney essence and kidney-yang deficiency; wet evil, blood stasis, pathogenic wind, cold pathogen and heat evil are important conditions; the treatment principles include nourishing kidney essence, warmming kidney-yang mainly, expeling wind, cold (heat) and dampness, and dredging channels at the same time, together with dredging blood and alleviating pain. Nourishing kidney drugs are effective for AS, and the side effects are Less. This study has a reference value on treating AS by TCM, highlights the efficacy and safety advantage of nourishing kidney drugs for AS.
