Background:The dispute of the superiority of laparoscopic liver resection(LLR)compared with open liver resection(OLR)is a heated discussion.We aimed to perform a systematic review to evaluate the superiority between LLR and OLR.Methods: The studies that compare LLR and OLR on patients undergoing liver resection were searched and a meta-analysis was performed according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)protocols.Results: This study included 40 observational studies with a total of 4,734 participants.Compared to OLR,LLR was superior in significantly decreasing postoperative complications,the length of the postoperative hospital-stay,and the operative mortality rate,for both primary liver cancer(PLC)patients and colorectal cancer liver metastasis(CRCLM)patients.Additionally,the long-time effects of LLR were not inferior than OLR.The subgroup analysis showed that liver cirrhosis and tumour size did not influence the short outcomes of LLR.Moreover,the R0 resection rate of LLR was higher than OLR for CRCLM patients.Conclusions: LLR provides patients with better perioperative outcomes than OLR.LLR is as safe and feasible as OLR,even for large liver tumours.
