| Background:Abnormal bone metabolism after endocrine therapy for breast cancer is a common complication.How to improve and regulate bone microenvironment homeostasis to prevent and treat osteoporosis is one of the clinical research hotspots.Traditional Chinese medicine(TCM)believes that the majority of the patients are deficient in liver and kidney.It has been clinically confirmed that the therapeutic effect of the formula of supplementing liver and kidney in the treatment of osteoporosis in this stage is accurate,but no breakthrough has been made in the exploration of the relevant treatment mechanism.Clinical application,we found that guided by the method of benefit hepatorenal tortoises deer when three conditions for improving bone metabolic abnormalities,including three yellow decoction can affect the breast malignant tumor patients within the environment of the tumor cells in the body,reduce the serological related indicators of oxidatie stress levels in the body and at the same time have anti-inflammatory,improve the related parameters of the body and blood stasis,and enhance the overall antitumor efficacy,tortoise plastron glue and antler glue is the product of flesh sentient beings,have good add filling pulp,nourishing liver and kidney function,add the tortoise plastron glue and antler glue three yellow decoction,form a turtle when three deer,guidelines on two virtual endocrine therapy of breast cancer patients with liver and kidney,It was further speculated that its mechanism of action was related to the regulation of the inflammatory mediatorsIL-2,IL-4,IL-8,IL-10,IL-1,TNF-α,and the collagen related factors PINP,β-CTX,and n-mid in patients,and its changes could be regulated on the basis of the regulation of NO,SOD,MDA and estrogen in patients’ stress.OBJECTIVE:to explore the mechanism and target of Guilusanhuang decoction(GLSD)in regulating bone microenvironment metabolism in patients with osteoporosis.Methods:The subjects were randomly divided into Control Group(N=30)and Treatment Group(N=30).The treatment group was treated with oral endocrine medicine plus Sanhuang decoction and Calch,while the control group was treated with simple endocrine medicine plus calch.The symptom scale of TCM,osteoporosis scale,quality of life scale,Kamofsky(KPS),KPS,IL-8,IL-10,IL-1,TNF-α,NO,Sod,MDA,CEA,CA125,CA153,PINP,β-CTX,N-MID,and bone mineral density were observed before treatment,3 months and 6 months after treatment Before treatment and 6 months after treatment,blood routine examination,liver and kidney function,electrocardiogram,color doppler ultrasound of liver,gallbladder,pancreas and spleen,chest CT were performed.Results:In this study,the abnormal bone metabolism of the patients with endocrine therapy of breast cancer with deficiency of liver and kidney was investigated.The control group took calcium tablets only,and the treatment group took Guilusanhuang decoction on the basis of calcium tablets,at the same time,blood routine examination,liver and kidney function,electrocardiogram,color doppler ultrasound of liver,gallbladder,pancreas and spleen,and chest CT were not abnormal.It was found that the overall improvement level of the treatment group was higher than that of the control group(p<0.05)after taking Guilusanhuang decoction for 6 months,which was consistent with the results of Chinese osteoporosis scale,the quality of Iife(Qol)of the treatment group increased after 6 months using Guilu Sanhuang decoction(p<0.05),especially the total scores of pain,physical function and mental state,physical activity and self-health evaluation were improved(p<0.05),which improved part of the patients’quality of life.KPS score was used to monitor the patients,tolerance to anti-tumor therapy,the score of the Treatment Group was higher than that of the control group(p<0.05),and the patients could tolerate the current treatment plan well.In the laboratory test,NO and SOD in the treatment group were lower and higher than those in the control group(p<0.05),which suggested that Guilu Sanhuang decoction could improve the oxidative stress of the patients In monitoring the level of inflammatory factors in patients,the level of IL-2 in the treatment group was higher than that in the Control Group 6 months after treatment(p<0.05),and the level of 11-10,IL-1 and TNF-α were lower than that in the control group(p<0.05),in order to confirm the intervention of Guilusanhuang decoction on abnormal bone metabolism,the indexes of bone metabolism and bone mineral density were measured,and it was found that after 6 months of treatment in the treatment group,pINP and β-CTX were lower than that before treatment(p<0.05),which suggested that Guilusanhuang decoction could improve the bone synthesis and bone resorption,and the bone mineral content of the treatment group increased after 6 months treatment(p<0.05)During Tumor Marker Monitoring,CEA in the treatment group was significantly improved(p<0.05),and the level of estrogen(p>0.05)in the treatment group was not significantly changed(p>0.05),it is suggested that the addition of Guilu Sanhuang decoction has no effect on endocrine therapy.Conclusion:Guilusanhuang decoction can improve the related inflammatory factors and reduce the oxidative stress reaction microenvironment,reduce the abnormal metabolism of bone decomposition,reduce bone absorption,increase bone formation,and does not affect the effect of endocrine therapy. |
