Objective: To explicit the protective effect of ShenQi compound on gastrocnemius muscle of GK rats,explore the potential mechanism and to provide scientific evidence for treatment of diabetic sarcopenia by invigorating Qi and spleen with promoting blood circulation.Methods: Choose the GK rats which random blood sugar is over11.1mmol/L,and feed them with high-fat diet for 4 weeks to copy the diabetes modal.The rats were assigned into three groups randomly: modal group(M),ShenQi compound group(Z),as well as Sitagliptin group(Y),10 rats in each group.Another 10 wistar rats are taken as normal control group.There are 4 groups in total.The intervention period is 8 weeks.During the intervention period,water intake,food intake,weight and blood glucose are monitored.At the end of intervention,blood was drew from abdominal aorta and the following indicators are evaluated: blood glucose,insulin,blood lipid,insulin-like growth factor-1(IGF-1),C reactive protein(CRP),tumor necrosis factor alpha(TNF-α)and interleukin 1β(IL-1β).Homeostasis model assessment-insulin resistance(HOMA-IR)index is calculated.Musculus gastrocnemius of rats are weighted and dyed with HE,and the morphological alterations are observed.Q-PCR technology is utilized to assess the mRNA expression of Pik3r1、Akt1、mTOR,while western blot technology is utilized to assess the protein expression of PI3 Kinase p85 alpha、mTOR(phospho S2448)、S6K1(phospho T389).Results:1.ShenQi compound improves general condition of GK rats,reduces drinking and food intake;2.ShenQi compound may contribute to maintain Gastrocnemius muscle wetweight of GK rats(P<0.01),as well as reduce atrophy,edema and rupture of skeletal muscle cell,reduce inflammatory infiltration of muscle tissue;3.ShenQi compound may lower blood glucose and lipid level of GK rats(P<0.01),increase insulin excretion(P>0.05).However,insulin resistance is not well improved both in ShenQi compound group and Sitagliptin group(P>0.05);4.ShenQi compound may lower CRP,TNF-α and IL-1β level in blood circulation(P<0.01),improve inflammatory condition;5.ShenQi compound may improve IGF-1 level in blood circulation(P<0.01);6.ShenQi compound may improve Pik3r1、mTOR mRNA expression(P<0.05),increase PI3 Kinase p85 alpha、mTOR(phospho S2448)、S6K1(phospho T389)protein expression(P<0.05).Conclusion: ShenQi compound may improve the general status of GK rats,reduce water intake and food intake,lower the body weight,maintain wet weight of musculus gastrocnemius,regulate blood glucose and lipid metabolism disorder.ShenQi compound may relieve muscle cell atrophy,edema,rupture and inflammatory cell infiltration condition.The effect of ShenQi compound on skeletal muscle maybe rely on it’s impact on IGF-1,TNF-α and IL-1β.The modulation of PI3K/Akt/mTOR signaling pathway may also be related to ShenQi compound’s protective effect on skeletal muscle.This study reflects multiple treatment targets of Chinese Traditional Medicine,enriches the content of classical TCM theory“spleen governs muscle”,as well as provides scientific evidence for treatment and prevention diabetic related sarcopenia with Chinese herbal medicine.
