Clinical Study On Treatment Of Type2 Diabetes Mellitus With Fatty Liver Qi And Yin Deficiency By Phlegm And Blood Stasis Syndrome

Purpose:Through literature study and summary of Chinese medicine and Western medicine for type 2 diabetic patients with fatty liver,observe the effect of Shenmai Lanling through clinical studies in patients with clinical efficacy and safety of treatment of type 2 diabetes mellitus with fatty liver Qi Yin deficiency with phlegm and blood stasis syndrome,to explore the mechanism of the prevention and treatment of this disease.Methods:According to the diagnostic criteria of Western medicine with fatty liver in type 2 diabetes,TCM is Qi Yin deficiency with phlegm and blood stasis,with more than two selected clinical diagnostic criteria of 60 patients were observed,randomly divided into treatment group and control group.30 patients in the control group were treated by conventional western medicine treatment;30 patients in the treatment group received routine treatment of Western medicine,combined with Shenmai Lanling Fang decoction,daily a quote,sooner or later oral,two groups were observed for 12 weeks.Main outcome measures:(FBG,P2hBG,HbAlc),fasting serum insulin(FINS),clinical symptoms and signs of traditional Chinese medicine(TCM)were significantly correlated with the changes of blood glucose(FBG,P2hBG,HbAlc),blood lipid(TC,TG,HDL-C,LDL-C)While assessing the patient’s adverse reactions and compliance.Results:Baseline comparison,the treatment of the two groups before and after the age and course of disease,after statistical analysis,no significant difference(P>0.05),comparable.(1)Blood glucose index(FBG,P2hBG,HbAlc):Treatment group and control group before and after treatment in patients with FBG,P2hBG and HbA1c decreased significantly,the curative effect of the treatment group had a significant difference(P<0.05);the FBG of the treatment group improved compared with the control group,there were highly significant differences(P<0.01),P2hBG and HbAlc improvement group there is a significant difference compared with the control(P<0.05);the two group in sugar analysis,HbA1c,FBG and PBG were significantly lower than those before treatment(P<0.05);comparison between groups,the curative effect of the treatment group had a significant difference(P<0.05).(2)Blood lipid,liver function:Treatment group and control group were decreased after treatment of TG,TC and LDL-C levels,compared with before treatment were significantly different(P<0.05 or P<0.01);comparison between groups,the treatment group TG,TC,HDL-C,LDL-C treatment had a significant difference(P<0.05).The liver function,the treatment group and the control group,ALT,AST,GGT were significantly different than before treatment(P<0.05).There was significant difference between the two groups(P<0.05).(3)Part of TCM syndrome curative effect:The effective rate of treatment group was 90%,significantly better than the control group 66.67%,there are significant differences(P<0.01);the two group compared with the symptom score than before treatment there were significant differences(P<0.05 or P<0.01);comparison between groups,the curative effect of the treatment group had a significant difference(P<0.05).(4)Fasting insulin,HOMA-IR,liver B:Two groups of patients before and after treatment,the treatment group and the control group HOMA-IR was significantly lower than before treatment(P<0.05 or P<0.01);comparison between groups,the curative effect of the treatment group had a significant difference(P<0.05).After treatment,fasting insulin decreased in the treatment group,and there was a very significant difference(P<0.01)compared with before treatment.There was no significant difference in fasting insulin between the control group and the control group(P>0.05).After treatment,there were significant differences in the therapeutic effect of liver ultrasound in the treatment group,and there was no significant difference between the control group and the B-mode ultrasound(P>0.05)in the control group(P<0.05).Between groups,the treatment group and the control group in the reduction of B ultrasound indexing.there are significant differences in efficacy(P<0.05).Conclusion:From the results of clinical studies,Lanling Shenmai Decoction in the treatment of type 2 diabetes with fatty liver,effectively improve the clinical symptoms;can reduce blood glucose and glycosylated hemoglobin,decrease blood lipid values,enhance the basic metabolic rate and reduce insulin resistance and improve blood rheology,and no damage to liver and kidney function,clinical curative effect is safe and reliable.
