BackgroundLots of reasons can cause breast soft tissue loss, which will make breasts? size and shape too small, or make people unconfident. As one of the most common plastic surgery,the breast augmentation surgery has been widely adopted to improve the morphology of the breast. Because of the good plasticity, no rejection and less damage, autologous lipotransfer has been used in soft tissue augmentation. However,the uncertain survival rate of grafted fat limited its application.With advancement of fat harvesting, preparation and injection, the survival rate of grafted fat has been improved. The aim of this study was to identify the safety of the procedure by means of a systematic review of clinical complications, radiographic changes and incidence of breast cancer(primary or recurrent) in women treated with fat grafting to the breast. To explore an effective way to increase fat retention, reduce complications and better breast radiography. MethodsA comprehensive literature search of PubMed(1938 to 20l4 years) was performed, and all the resulting articles were screened and reviewed. ResultsA total of 3037 cases of 15 articles dating from 1938 to 2014 were included in this study. There was significant heterogeneity(P <0.1, I ^ 2 = 93.1%) among the studies including the complication rate. The clinical complication rate of the autologous fat breast augmentation surgery was 12.05% 12.05% [95%CI(7.47%; 19.51%)], including 145 cases of palpable nodule(56.0%), 69 cases of cyst(26.6%), 17 cases of infection(6.6%), 14 cases of dysaesthesia(5.4%), 12 cases of haematoma(4.6%), 2 cases of pneumothorax(0.8%). A total of 400 abnormal radiological findings(13.17%) were identified on mammography during follow-up.ConclusionsAccording to this study, there is wide-spread clinical applicability of autologous fat grafting for breast reconstruction. Complications were few and no evidence showed that the procedure has increased the post-operative incidence of breast cancer. The most common complications were palpable nodules and cysts, which were associated with the necrosis of grafted fat.
