| ObjectiveThe subject of clinical research was used the randomized and controlled by experimental methods. Three different combined therapies which are conve-ntional medicine (Celeoxib, and Shentong Zhuyu Decoction), electroacupunctur e-spine collaterals cupping, and electroacupuncture-spine collaterals cupping combined conventional medicine (Celeoxib, and Shentong Zhuyu Decoction) are used to observe the clinical therapeutic effect of knee osteoarthritis of blood stasis in collaterals, and to appraise through comparison.The studies expect ot explore the best combined therapy and the mechanism of actions that can provide the references and applies of the treatment of this disease.MethodsThe studies collect 90 cases of nonoperative management that come from orthopedics and acupuncture clinic and wards of the Affiliated Hospital of Traditional Chinese Medisine University of Guangzhou and other region hosp-itals. These patients are devided into 3 treat groups, the first controlled group that treated by conventional medicine, the second controlled group that treated by electroacupuncture-spine collaterals cupping, and the experimental group that treated by electroacupuncture-spine collaterals cupping combined conventional medicine. The collection time of the case of illness is from February 2015 to April 2016.The treatment scheme is three times a week, and the treatment course is one month. When the treatment course is over, the therapeutic evaluation is done immediately. The items of therapeutic evaluation include SF-36 quality of life assessment, WOMAC knee rating scale, JOA knee score, and VAS score.Before the proceeding of the research, all of the clinical basic inform-ations of all groups that include gender, age, SF-36 quality of life asses -sment, WOMAC knee rating scale, JOA knee score, and VAS score are analysised by statistic methods. It is necessary to confirm that there are no significant differences among all groups, (p>0.05) and the studies have the comparabi-lity, and then the studies can be carried on.Results1.The comparison of SF-36 quality of life of all groups after treatment According to the research results of SF-36 quality of life (table.2) after treatment, The comparison of quality of life of all groups after treatment, generally speaking, the experimental group is the optimal, the second contro-lled group takes second place, and the first controlled group follow by.2. The comparison of clinical symptoms and physical signs of all groups after treatment(1) According to the research results of WOMAC knee rating scale (table.3) after treatment, generally speaking, the curative effect of the experimental group is the best,but the second controlled group and the first controlled group have the therapeutic equivalence, and the dufference between the both is a little (p>0.05).(2) According to the research results of JOA knee score (table.4) after treatment, generally speaking, the experimental group is better than the first controlled group and the second controlled group in therapeutic effect, but the first controlled group and the second controlled group have the therapeutic equivalence, and the difference between the both is very little (p>0.05)(3) According to the research results of VAS score (table.5) after treatment, generally speaking, on the aspect of the improve effect, the second experimental group is better than the first controlled group and the second controlled group, and both of the first controlled group and the second controlled group have the therapeutic equivalence, the difference between them is approximated (p>0.05)3.The comparison of clinic curative effect evaluation after treatmentThe curative rates of treatment, the first controlled group is 65.52%, the second controlled group is 68.97%, and the experimental group is 82.76%, the total effective rates of treatment, the first controlled group is 86.21%, the second controlled group is 89.66%, and the experimental group is 93.10%. In the regard of curative rate of treatment, the first controlled group and the second controlled group have no obvious difference (p>0.05), but the experimental group has obvious difference clearly (p<0.05). It is showed that the experimental group has the optimal efficacy than other groups. In the regard of total effective rate of treatment, after the statistic analysis of H rank-sum test, H=6.25, p=0.15, the total effective rates of treatment of all have no the significant difference (p>0.05). It points out that all groups-that treat the knee-osteoarthritis have the affirmative effect in their treatments.ConclusionThe studies are proceeded to treat the knee osteoarthritis of blood stasis in collaterals by three therapies which are conventional medicine (Celeoxi-b, and Shentong Zhuyu Decoction), electroacupuncture-spine cupping, and electroacupuncture-spine cupping combined conventional medicine (Celeoxib, and Shentong Zhuyu Decoction), and to do the research of clinical effect. Four kinds of assessment method that are SF-36 quality of life, WOMAC knee rating scale, and JOA knee score, and VAS knee score are used to proceed the evaluation in the front of treatment and the ending of treatment. About the results of overall evaluations, in the regard of rating scale, JOA knee score, and VAS knee score are used to proceed the evaluation in the front of treatment and the ending of treatment. About the results of overall evaluations, in the regard of the improvement of quality of life, the integral symptoms and ph-ysical signs, the therapy of the experimental group that use electroacupunc-ture-spine cupping combined conventional medicine (Celeoxib, and Shentong Z-huyu Decoction) has the best improvement effect. But the improvement effects of the first controlled group that use conventional medicine (Celeoxib, and Shentong Zhuyu Decoction) and the second controlled group that use electro-acupuncture-spine cupping are equivalent. In the regard of the comparison of therapeutic effect after treatment, in the term of curative rate of treatment, the experimental group has the optimal effect obviously, and in the term of the total effective rate of treatment, all of the groups have no the significant difference.It is hinted that the therapies of all groups are all effective.The results of the studies can provide the direction of new thoughts and the application references for the treatment of knee osteoarthritis. Expecting the superiority and the characteristic of the acupuncture combined with medication can give full play to its incremental effect, and making the therapeutic scheme can be more perfect, and the curative effect can be more positive. |
