Ligamentum Flavum by consist of elastic fibers andcollagen fibers,is good for maintain spinal stability,and which is the important structure ofback of it.ligamentum flavum degeneration can cause the decreased elasticity, when spinestretched ligamentum flavum can appear ruffle or fold and into the canalis spinalis, makethe canalis spinalis volume decreases, and lead to spinal cord, nerve root under pressure,neurological symptoms appeared, cause paralysis seriously.Studies confirmed that the inflammatory factor TNF-α, IL-1, IL-6, TGF-β1and thedegeneration of the ligamentum flavum, expression in hypertrophic ligamentum flavum inthe form of a relative consistency, about80%significantly increased, with inducing orpromoting effect on the clinical symptoms of patient. Diacerein was extracted fromrhubarb anthraquinone derivatives, metabolites of Rhein inhibited inflammatory andmetabolic abnormality of cytokines such as IL-1, IL-6, TNF-α, and played a goodanti-inflammatory, analgesic effect, especially IL-1; IL-1, IL-6, TNF-alpha and TGF-beta1process involved in degenerative changes of the ligamentum flavum hypertrophy, andinduced or promoted the degeneration and hypertrophy. Diacerein had a certain role on theyellow ligament hypertrophy of the degeneration process. Application of diacerein after4-6weeks, the curative effect lasted at least2-3months after stopping, diacerein in longhalf-life in vivo.The subject used Diacerein treatment in the degeneration of the yellow ligamenthypertrophy process, from morphological, molecular biology to explain the mechanism ofaction. It may provide a new therapeutic approach for the non-surgical treatment ofpatients with spinal stenosis caused by yellow ligament hypertrophy. There is not a study of diacerein on the degenerative and hypertrophy changes of the ligamentum flavum. Mostof the studies focused on the inflammatory factor effect on degenerative changes of theligamentum flavum hypertrophy. This research focused on inflammatory cytokineinhibitors in the degenerative hypertrophy changes of the ligamentum flavum This study isto investigate the mechanism action of diacerein on cell and fiber yellow ligament, toexplore the prevention and treatment possibilities of application of diacerein hypertrophyof ligamentum flavum, to find the method of conservative treatment of lumbar spinalstenosis. |
