| Objective To investigate the clinical value of magnetic resonance Imaging in evaluating right and left ventricular function of patients with Idiopathic Dilated Cardiomyopathy (IDCM).Methods MRI was performed in 42 (32 men, 10 women; age range, 28~74 years; mean age, 54.81±13.99 years) patients with IDCM, and in 44(23 men, 21 women; age range, 25~80 years; mean age 52.91±11.02 years) normal subjects as the control group were processed using an ARGUS auto-quantitative Program by using a GE Sign 1.5-T scanner with cine FIESTA and a phased-array body coil. New York Heart Association (NYHA) functional: classâ… ~â…¡11cases , classâ…¢16 cases , classâ…£15cases。Time-volume-curves of left ventricular and right ventricular were evaluated and some indexes were calculated. The study compares data of the right and left ventricular measured by MRI between the patient group and the control group was calculated.Results There was a significant difference between patients group and the control group in sex and heart rate. There was a significant difference between patients group and the control group in parameter of cardiac function too(P<0.0001), the measurements of ventricular EF,LVPER and LVPFR were smaller than those in the control group, the measurements of ventricular EDV,ESV,LVEDD,LVLAD and MM in patients with IDCM were larger than those in the control group. The measurements of wall thickening ratio in patients with IDCM were significantly smaller than those of the control group. The correlation was good and with significantly higher values with the class of NYHA functional.Conclusions MRI is a fast comprehensive technique for the assessment of cardiac volumes and function in IDCM because of its accuracy but also its high reproducibility. The correlation was good for the class of NYHA functional. Objective Left ventricular function and volumes provide valuable diagnostic information and are prognostic importance in patients with Idiopathic dilated cardiomyopathy (IDCM). This study was designed to compare 2D-echocardiography,gated single-photon emission computed tomography (SPECT) and cardiac magnetic resonance imaging (MRI) for evaluation of these parameters in patients with IDCM. Methods cardiac MRI,Gated SPECT and 2DE were performed in 42 patients (32 men, 10 women; age range, 28~74 years; mean age, 54.81±13.99 years) having IDCM.Left ventricular function and volumes were measured using Simspon for 2DE,QGS software for gated SPECT,and an ARGUS auto-quantitative Program for MRI.Results of 2DE and SPECT were compared with MRI. New York Heart Association (NYHA) functional: classâ… ~â…¡11cases,classâ…¢16 cases,classâ…£15 cases. Results The correlation gated SPECT( r≥0.9, P≤0.001),2DE (r = 0.699~0.878, P≤0.001) and MRI was good for the assessment of LVEF,EDV and ESV. There was a significant difference between MRI and SPECT,2DE in estimated volumes, but EDV and ESV were underestimated by SPECT and 2DE. The mean MRI EF was underestimated by 2DE, but not by SPECT. MRI with NYHA class: ESV,SV,EF,PER,PFR,EDD and ESD of class III/IV compared with class I~II had significantly mean;2DE with NYHA class: ESV and EF of class III/IV compared with class I~II had significantly mean, EDD and ESD IV compared with class I~II had significantly mean; SPECT with NYHA class: ESV and EF of class III/IV compared with class I~II had significantly mean.Conclusions Gated SPECT and 2DE correlate well for the assessment of left ventricular function and volumes. However,both 2DE and SPECT underestimated LV volumes. Moreover, functional analysis with MRI was more advantage of providing information about left ventricular function,volumes than that with 2ED or SPECT. The correlation was good for the class of NYHA functional. All patients and the control group underwent resting myocardial gated SPECT, 60min after injection of 99Tcm–MIBI 925MBq, were processed using QGS software for gated SPECT. The data, including left (LV) and right ventricular (RV) function SPECT were compared with those estimated with MR imaging.Results: There was a significant difference between patients group and the control group in parameter of cardiac volumes. The correlation was good delayed-enhancement MRI and perfusion defect SPECT in ICM.There was a significant difference between ICM group and the IDCM group in parameter of left ventricular function and volumes in EDV,ESV,EF,MM,PER and PFR(P<0.0001); The measurements of right ventricular function were smaller in IDCM than ICM,but, There was a significant difference between ICM group and the IDCM group; Good correlations were observed between MRI and gated SPECT for EDV,ESV and EF (r values ranging from 0.663 to 0.765).Conclusion: Gated SPECT and MRI correlate well for the assessment of ventricular function and volume,with MR myocardial delayed enhancement , SPECT and MRI can help to discriminate ischemic cardiomyopathy from the differential diagnoses of Idiopathic dilated cardiomyopathy. For correct recognition of such patients, we compared the findings in cardiac MRI between HCM and Idiopathic dilated cardiomyopathy (IDCM) patients.Methods 9 patients with HCM (6 men, 3 women; age range, 30~72 years; mean age 44.57±14.90years), 42 patients with IDCM (32 men, 10 women; age range, 28~74 years; mean age, 54.81±13.99 years), and 44 normal subjects (23 men, 21 women; age range, 25~80 years; mean age 52.91±11.02 years) as the control group were processed using an ARGUS auto-quantitative Program by using a GE Sign 1.5-T scanner with cine FIESTA and a phased-array body coil. Time-volume-curves of left ventricular and right ventricular were evaluated and some indexes such as Myocardial Mass(MM),Ejection Fraction(EF) and Myocardial Thickening(MT), peak ejection and filling rates (PER, PFR) time to PER and PFR (TPER, TPFR) were calculated. The study compares data of the right and left ventricular measured by MRI in HCM, IDCM group and the control group was calculated.Results There was no difference between patients group and the control group in Clinical characteristics. There was a significant difference between HCM group and the control group in parameter of cardiac function (P<0.0001), the measurements of ventricular EF,PER were bigger than those in the control group, There was a significant difference between HCM group the measurements of ventricular EDV,ESV,EF,PER,PFR and MM in patients with IDCM. There was no difference between HCM group and IDCM group in right ventricular faction. The measurements of wall thickening ratio in patients with IDCM were significantly smaller than those of the control and HCD group.Conclusions MRI is a useful imaging tool for the diagnosis of HCM because it can readily diagnose those with function and anatomy. In addition, MRI can help to discriminate HCM from the differential diagnoses of Idiopathic dilated cardiomyopathy. |
