| Objective:To uncover the relevant TCM theories of physique and CP (chronic prostates),summarize the research fruits of recent and modern times, explore the effects of malephysique characteristics upon the andrological diseases and understand the effects of physiquecharacteristics upon the etiology/pathogenesis, diagnosis/treatment, prognosis and outcome ofCP. At present, there is still no report of any study involving TCM physique characteristicsand CP. Further no focused epidemiological survey is available. The present study wasdesigned to employ the clinical epidemiological methods to obtain the distribution statisticfrequencies of CP TCM physique types, analyze the distribution patterns of TCM physiquecharacteristics and explore the correlation strength of TCM physique types and biologicalparameters, CPSI and IEF5 so as to determine the underlying linkage of physiquecharacteristics, biological parameters, symptoms, psychology and erectile functions of thisdisease and enrich the contents of CP syndrome-based differentiation and establish a scientificsystem of CP syndrome-based differentiation. It is a new thought and method to study CPfrom the viewpoints of TCM physique so that the TCM characteristics of CP will becomemore complete and more comprehensive theoretic basis and guiding thoughts will be providedfor TCM ways of diagnosis, treatment, health-maintaining, rehabilitation and prevention.Methods: The methods of traditional literature rearrangement and modern literature analysiswere used to review the last several decades' understanding of etiology, pathogenicmechanism, diagnostic classification, clinical characteristic, TCM differentiative types andtherapeutic progress of CP-related Chinese & Western medicine; The application of TCMphysique theory in diseases was explored; The effects of male physique characteristic uponandrological diseases were examined; The effects of physique characteristic upon theetiology/pathogenesis, diagnosis & treatment, prognosis and outcome of CP were observed.The clinical epidemiological method was used to observe 555 patients and themulti-parameter derivative variable statistic analysis technique employed to analyze physiquecharacteristic distribution patterns, biological parameters, multi-parameter correspondenceand correlation between CPSI and IEF5 for the CP patients.Results:①The distribution of CP physique characteristic was as follows: blandness harmonycondition 76 cases (13.7%), Qi deficiency condition 65 cases (11.7%), Yang deficiencycondition 63 cases (11.4%), Yin deficiency condition 44 cases (7.9%), phlegm dampcondition 65 cases (11.7%), moist heat condition 83 cases (15%), blood stasis condition43 cases (8%), Qi stagnation condition 78 cases (14%), special disposition condition 38 cases(7%);②Qi stagnation condition (charge ratio of 2.414493 ) and severe CPSI (charge ratioof 2.652597), the correlation was the largest;③moist heat condition(charge ratio of 0.1887)and mild (i.e. WBC 20~50 count) (charge ratio of-0.5365) had a shorter distance with alarge correlation. The next was Qi stagnation condition (charge ratio of 0.2053) and mild (i.e.WBC10~20 count) (charge ratio of 1.307) with a large correlation;④moist heat condition (charge ratio of 0.599793) and lecithin corpuscle++ (charge ratio of 3.327131), thecorrelation was the largest. Blandness harmony condition (charge ratio of 0.299132) andlecithin corpuscle+++(charge ratio of 0.09541) had a shorter distance with a largecorrelation;⑤moist heat condition (charge ratio of 3127) and mild ED (charge ratio of4.056), the correlation was the largest; Yin deficiency condition (charge ratio of-0.3281 )and moderate ED (charge ratio of-1.2485 ), the correlation was the largest; Yang deficiencycondition (charge ratio of-0.9743) and severe ED (charge ratio of-0.0997), the correlationwas the largest;⑥Qi deficiency condition (charge ratio of-4.972 ) and above college (chargeratio of 64.514), the correlation was the largest;⑦special disposition condition (chargeratio of-7.045) and students (charge ratio of 8.5924), the correlation was the largest;⑧Qideficiency condition (charge ratio of 2.110865 ) and being married (charge ratio of-0.61383),the correlation was the largest.Discussion: As indicated by the literature researches, despite of a considerable degree ofknowledge of pathogenic mechanism of CP based upon modern medicine, there is still nobreakthrough progress. Most modern TCM studies approached from the therapeuticviewpoints and lacked a systemic study of such a multitude of aspects as etiology,pathogenesis, symptomatic features, physique characteristic, psychology and sexology fromthe perspectives of TCM.As viewed form the theoretic levels of TCM physiques, the male physique characteristic hasimportant influences over the andrological diseases. The main manifestations included themales utilize Yang & Qi, treat sperms as fundamental, male ejaculation with replenishing andlosing and the close relationship of male ejaculation and physiques, etc. The physiquecharacteristics have important influences over the etiology/pathogenesis and diagnostictreatment of chronic prostatis. The syndrome-based differentiation is of key importance totreating the causes and preventing recurrence of chronic prostatis.As demonstrated by the clinical epidemiological studies, the physique basis of CP patientswas overwhelmingly of solid physique (phlegm damp condition, moist heat condition, phlegmblood condition and Qi stagnation condition). It illustrated that the pathogenesis of CP hadtransformed into the mixture of emptiness and solidness with the predominance of the latter;The patients with Qi stagnation condition tended to show more severe clinical symptoms; Thepatients with moist heat condition were inclined to have a higher WBC count in the prostaticfluid; Blandness harmony condition had some protective effects over CP. The occurring basisof mild ED in CP patients was predominantly of solid physique. The chronic prostatespatients with empty physique were more likely to suffer moderate and severe ED. |
