| Satisfactory surgical reconstruction of the external ear is still a major challenge for the plastic surgeon. In this paper, we reviewed the relating literatures. The whole auricular defects by developing of the method reconstruction with autogenous tissue and reconstruction with auricular prostheses. The criteria for treatment selection, the advantages and disadvantages of each approach were considered.The methods of auricular reconstruction with autogenous tissue are various. But they all include two basic factors: framework fabrication and the skin and soft tissues covering the framework. This kind of method is step by step perfect in 10 to 20 years.The technique of skin expansion is a useful way to solve the problem of the mastoid region skin is not enough to cover the framework. Ear framework fabricated with autogenous rib cartilage is still the most perfect today. As for some people that their rib cartilages are not suitable for transplant. The Medpro is a good selection because it is a kind of favorable biocompatible materials. Two major problems must be solved with osseointergrated implant for replacement of absent or defective ear. One is marking of auricular prostheses. The other is retention auriaular prostheses. It is essential that the patient feel so confident that his prostheses will stay in place regardless of activity.
