【Objective】 To study the change of regional cerebral metabolism rate of glucose (rCMRglc) in adults and the relationship between CMRglc and age or sex;To study the feature of rCMRglc in patients with mild cognitive impairment (MCI) and in patients with Alzheimer's disease (AD) by positron emission tomography (PET) and its relationship with neuropsychological test;To study the relationship between MCI and AD; To study the risk factors of behavioral and psychological symptoms of dementia (BPSD), and to compare the feature of PET in patients with and without BPSD.【Methods】 18F-fluoro deoxyglucose -Positron emission tomography scanning (18F-FDG- PET) was conducted in 28 health volunteers (HC group, 17 below 60 years as HC1 group and 11 above 60 years as HC2 group); PET,MMSE and WMS were conducted in 10 patients with MCI ,10 health volunteers as a control group (HC3) and 13 patients with AD. Dementia behavior disturbance scale (DBD) and behavioral pathology in Alzheimer's disease rating scale (BEHAVE-AD) were used to measure the degree of BPSD of AD. PET was conducted in 13 patients with AD, of those 6 with BPSD and 7 without BPSD.【Results】 1. Results of brain PET in health adults: (1) The rCMRglc of frontal lobe, orbital gyrus, anterior and posterior cinggulate gyrus, hippocampus and parahippocampal gyrus, caudate nucleus, putamen, temporal lobe, thalamus, insular lobe, lateral tempo-occipital gyrus, postcentral gyrus, parietal lobe, temporparieto-occipital (TPO), amygdaloid body, left medial occipitotemporal gyrus and left precentral gyrus was lower in HC2 group than that in HC1 group (p<0.05~p<0.001). (2) The rCMRglc of left inferior temporal gyrus, right and left precentral gyrus had or no relationship with aging. The rCMRglc of other parts of brain was correlated negatively with aging (p<0.05~p<0.001). All parts of brain we measured had no relationship with sex(p>0.05). 2. Results of brain PET and neuropsychological test in MCIpatients: (1) Scores of MMSE and WMS achieved by MCI and AD patients were lower than those by HC3 (p<0.01). (2) The rCMRglc of the left orbital gyrus, the right middle temporal gyrus and the left caudate nucleus was lower in MCI group than that in HC3 group (p<0.05). (3) The rCMRglc of many parts of brain such as orbital gyrus, left superior frontal gyrus, left inferior frontal gyrus, middle temporal gyrus, inferior temporal gyrus, left parietal, left precentral gyrus, right postcentral gyrus, left medial occipitotemporal gyrus, lefthippocampus, left parahippocampal gyrus, right anterior cingulate gyrus, posterior cinggulate gyrus and left amygdaloid body was correlated negatively with age ( p<0.05 or p<0.01). The rCMRglc of left inferior temporal gyrus, precentral gyrus, left postcentral gyrus was correlated positively with years of education ( p<0.05 or p<0.01). The rCMRglc of left caudate nucleus was correlated negatively with years of education ( p<0.05). The rCMRglc of many parts of brain such as inferior frontal gyrus, left superior temporal gyrus, middle temporal gyrus, inferior temporal gyrus, left parietal lobe, left precentral gyrus, postcentral gyrus, medial temporal gyrus, anterior cingulate gyrus, left insular lobe, amygdaloid body and the left caudate nucleus was correlated positively with MMSE( p<0.05 or p<0.01). The rCMRglc of inferior frontal gyrus, left superior temporal gyrus, middle temporal gyrus, inferior temporal gyrus, parietal lobe, precentral gyrus, postcentral gyrus, left insular lobe, thalamus and left putamen was correlated positively with WMS( p<0.05). 3. Results of brain PET and neuropsychological test in AD patients: (1) Watched by naked eyes, encephalatrophy, especially temporal lobe atrophy, and unevenness distribution of radiation in cortex was seen in AD group. There was mild reduction of rCMRglc in HC3 group, while marked in AD group, especially in parietal lobe, then temporal lobe and frontal lobe.
