| Purpose:To prospectively evaluate and compare the postoperative efficacy and complication of TVT-Secur for female stress urinary incontinence at different follow-up time.Methords:Patients with SUI, who underwent TVT-Secur treatment in two hospitals from October 2008 to October 2009, were selected as subjects. Using preoperative and intraoperative data and postoperative complications, the objective therapeutic effect and subjective satisfaction at the different follow-up time were evaluated.Results:A total of 30 women participated in this study.25 were diagnosed as pure SUI, and 5 were diagnosed as stress predominant mixed urinary incontinence, age ranged 39-77 years old; body mass index (BMI) ranged 22.8-31.9 kg/m2; parity ranged 1-6 times; course of disease ranged 1-20 years; ICI-Q-SF score ranged 7-14 scores; abdominal leak point pressure (ALPP) ranged 29-141 cm H2O. All patients showed positive results in Mashall-marchett test, four cases underwent pelvic surgery before, no patient underwent operation for anti-incontinence, and no patient suffered bladder outlet obstruction. Patients presenting with other significant features of pelvic floor relaxation and being performed anterior and posterior colporrhaphy concomitant with the anti-incontinence surgery were excluded from this study. Except one case suffered vesical perforation in surgery and spent 30 min, the others spent 13-19 min in surgery; no case occurred intra-operative blood loss for more than 100 ml. Mild post-operative dysuria was seen in 2 patient. Patients were scheduled for follow-up at lmo,3mo,6mo,12mo, while the cure rate was 83.3% (25 patients),66.7% (20 patients),63.3% (19 patients) and 60%(18 patients) respectively and the overactive bladder (OAB) symptoms were evidence with 11 patients (36.7%),10 patients (33.3%),6 patients (20%) and 7 patients (23.3%) respectively.Conclusions:TVT-Secur is a simple procedure for the treatment of SUI. It is safe to use, and the complications are minimal. However, our study showed that with the follow-up time becoming longer TVT-Secur had a high recurrence rate of SUI, while from 3 mo to 12 mo the therapeutic effect was relatively persistent. But their long-term effects still need to be confirmed by large-scale clinical data and long-term follow-up.
