Objective (1)To summarize the disease names of traditional Chinese medicine ( TCM ), etiological factors, pathogenesis, characteristics of presentation and status quos of TCM therapy, about knee osteoarthritis by studying both TCM works and TCM journals. (2)To study the characteristics of distribution of symptoms, types of presentation and status quos of diagnosis and treatment of the cases retrospectively and the characteristics of distribution of symptoms, types of presentation and essential diagnosis of the cases with knee osteoarthritis prospectively. (3)To establish the standard TCM project for the diagnosis and treatment of knee osleoarthritis.Methods (1)The medcal literarures in ancient TCM works, modern TCM works and published on the traditional Chinese medicial journals, with the key words of Bi-presentation, Flaccidity Presentation, cold-type arthalgia and osteoarthritis, from 1994 to 2003, were searched and reviewed. Word files and SPPS data base were established with the data of disease names, Etiological factors, pathogenesis, presentation, iatric and nonconforming therapeutics. It was coordinated and analyzed with statistics. (2)In different levels hospital in Hunan province, case histories,which first diagnosis was knee osteoarthritis, were checked and approved from 12 random hospitals, from 1997 to 2093, and the data of demology, presentation, diagnosis and treatment were excerpted from the case histories.following it,the data of demology and presentation were investigated from cases,which were well established knee osteoarthritis, in 6 randoms hospitals. Then, SPPS data base were established with the data of disease names, Eliological factors, pathogenesis, presentation, iatric and nonconforming therapeutics. It was coordinated and analyszed with statistics. (3)To establish the TCM project for the diagnosis and treatment of knee osteoarthritis on the base of bcforchcad literatures and epidemiological survey. Results ?9 ancient works were investigated totally.The disease names which were similar with knee osteoarthritis were bone bi-snydrome, cold-type arthralgia and crane-knee arthritis. Their bases of onset were weakness of the liver and kidney and deficiency of qi-blood. and main pathologies were stagnation of wind-cold-wetness evil. The primary principles of treatments were supporting healthy-qi to eliminate pathogenic factors and treatments were coadministered to tendons and bones, The primary treatments wereadministrating orally Chinese materia medica. Warming and recuperating herbal medicine were most frequently administrated orally. There were 35 TCM works were investigated. With the western medcine disease names such as ostcoarthrislis, degenerative osteothritis, hypertrophy osteothritis and osleoblastosis, which were given homologous TCM names, such as bi-presentation, crane-knee arthritis and lumbar-thign pain. Weakness and sthenia of evil were its pathogenesis. Deficiency of healthy energy included deficiency of kidney-Yang, weakness of the liver and kjidncy and deficiency of qi-blood. Sthenia of evil included blood stasis, cold-wetness and phlegm. It was proposed that knee osteoarthritis should be treated with combinative treatments. The commonly used therapeutics included herbal medicine to be administrated orally and externally used, acupuncture and moxibuslion, massotherapy, functional exercise and administrated NSAIDs orally if necessary. There were 278 medical literatures have been searched and reviewed from TCM journals. Knee osteoarthritis was most frequently used by mordon medicine and vested in the categories of bi-presentation, flaccidity-presentation, cold-type arthralgia and crane-knee arthritis. It's primary Etiological factors were blood stasis, cold, external wetness, wind and asthenia of kidney. Majority thinked that knee-osteoarthritis clinically manifested simplex principal symptom or chief presentation with associated symptom.
