Objective To explore the clinical effectiveness and part therapeutic mechanism of Xinzhiliuanjian decoction, a representative prescription of Xiefeihuayu therapy, for treatment of Hypoxia pulmonary hypertension (HPH) caused by Chronic obstructive pulmonary disease (COPD) in acute stage.Method 60 cases of patients with HPH in acute stage were divided randomly into treatment group and control group. All the cases were treated with suck oxygen, Levofloxacin Injection, Aminophyline and Bromhexine tablet for 14 days. In additional, the 30 cases in treatment group were treated with Xinzhiliuanjian decoction, while the other cases in control group were treated with Nifedipine Controlled Release Tablets. Symptom and signs were measured and recorded every day and the related items were measured before and after treatment.Result The clinical controlled and remarkably effective rate in treatment group is 66.7%, which is same with that in controls group (P>0.05). Clinical symptom such as cough, expectoration, gasping were improved greatly than those in controls group. Furthermore, the proper, coating, and vessel of tongue were unproved, compared with that in controls groupsignificantly. The systolic pressure of pulmonary artery was reduced in treatment group, while the pressure of systemic circulatio were stable. And the whole blood viscosity, plasma viscosity, white blood cell and endothelins-1 were obviously decreased. Forced vital capacity, forced expiratory volume in first second, oxygen pressure of artery , carbon monoxide and the scores of QOL were increased obviously..Conclusion As a representative prescription of Xiefeihuayu therapy, Xinzhiliuanjina decoction had good effect on pulmonary circulation selectivity in HPH caused by COPD. The clinical observation suggested that it may adjust the expression of carbon monoxide and endothelins-1 in plasm, prevent the pneumoangiogram from vasoconstriction and reconstitution. As a result, it could slowing pathology pricess of HPH and improve QOL.
