The use of pure titanium for palatal plate of upper complete denture is an important indication for titanium as oral prosthetic material. In order to investigate the clinical performance in detail, analyse the possible influence of surface machining on clinical effect, and improve the property of titanium prosthesis, this paper evaluates the clinical effect of pure titanium palatal plate and polymethyl methacrylate(PMMA) palatal plate for upper complete denture, the influence of surface roughness and three superficial treatments on bacterium adhesion, stability of color, corrosion resistance, wettability and hardness in vivo. Part I: Clinical retrospective study of pure titanium palatal plate for upper complete denture. 11 edentulous patients, who had worn titanium palatal plate denture and PMMA palatal plate denture made by the same specialist for complete denture, were recalled to accomplish all the questions. The results show that the primary cause of complete denture remade is midline fractures, ill retention, bad adaptation, and the wear of artificial tooth. Most patients prefer titanium palatal plate denture, because compared with PMMA palatal plate denture, it has excellent fracture-resistance, less foreign body sensation, no metal taste, light weight, good adaptation and comfortable feeling. Some palatal plates have pigmentation and discoloration. Because individual difference and grind quality of the denture may influence the results, fluther external study should be carried. Part II: Clinical effects of pure titanium palatal plate for upper complete denture. In order to eliminate the influence of individual difference and grind quality of the denture on the results, two completely same upper denture( one palatal plate is made of titanium, the other is PMMA) was made for 8 edentulous patients. The results showed that titanium palatal plate denture had more 3.4?.1 g in weight and more 105 ?38g in retention force than PMMA palatal plate denture. Compared with PMMA, besides higher intension, almost all the patient felt titanium palatal plate denture less foreign body sensation, no metal taste, lighter weight, better retention, more comfortable, and easier to get acclimatized when first wearing the denture. After 6months, they felt titanium palatal plate denture better retention and lighter weight all the same, but foreign body sensation and comfort made no difference between the two dentures with the increase of adaptation. The main disadvantage of titanium palatal plate denture was :1 .The wrinkly configuration on polished surface reduced the comfort in some patients. 2.There were pigmentation and discoloration on the polished surface on a few dentures, but not such phenomena is found on tissue surface. The individual oral environment has influence on color stability. Therefore, titanium palatal plate denture is more suitable for patients whose upper complete denture fracture repeatedly or who was extra sensitive to complete denture. 7 PartLll Effects of surface roughness on titanium palatal plate denture (?
