| Study on Chinese Health Care System Reform under Perspective of the Core Values of Socialism Chinese health care system reform has been going forward under explorations and making progress with setbacks. Primary achievement was got ever since the new reform started in 2009 in that the value orientations were clear and healthcare coverage was enlarged. However, serious challenges emerged on the direction of health care reform in that: the conflictions between doctors and patients become more and more prominent; the nature of public beneficiary of government owned hospitals was in doubt; problems such as medical care difficult to afford, health insurance system facing up with pressures, hospitals feeling wronged, patients feeling poor, governments having difficulties, market failure and information asymmetry and so on counteracted the achievements of the reform. In the background that economic develops slower and social political systems remain reforming; the health care system reform of China has entered a crucial stage. To facilitate the new health care reform and solve the problem of difficult and expensive to see a doctor has become a key project for China to construct harmonized society and it is urgent to re-orient the core values for the health reform in explorations. There are many researchers focus on core values of socialism and some other academics are involved in the theories of health reforms. But studies on health care reform from perspective of core values of socialism are scarce. Reforms on health care were more in terms of social medicine and health service management, but few people are viewing health problems standing outside the health system. Basing on literature review, theoretical analysis, historical study and case studies, from the viewpoint of scientific socialism theory and the core values of socialism and society development in terms of equality, justice and equity, the objectives of this study are as follows: Study on theoretical basis of values of health care reform in China, analyze the establishment, development, deficiency and returning, discuss the theoretical basis of health care reform based on the core values of equality and justice, and search the theoretical studies on health equities. Review the processes of health care system reforms ever since the establishment of the new China of 1949 and conduct comprehensive, periodical and specific historical evaluations on health reform of China, including health reform practice in Su Qian of Jiang Su Province. The studies find that although the objectives of values of public beneficiary for the new health reform was set up, the approaches and achievements of reform in different areas varied. We analyzed the role of the core values in health reform, summed up the deficiencies caused by lack of values and summarized the problems and its rooted causes. The research findings are: China has laid good foundations for the establishment of the core values of socialism and political basis of health reform, whilst the primary task currently is to set the core value in the leadership position because the core values are the necessity for health service development, will determine the nature and direction of health services, orient the objectives of health services of China and closely relate to the realization of the function and obligations of health care reform. This is also the outstanding characteristic of this study. Through typical case studies on comparisons and analysis of developed and developing countries in terms of incentives, practices, values orientation, taking references of international experiences, this study discovered: the establishment and perfection of health care system not only rely on economic development, but also is determined by the political willingness and value orientation of the society. Chinese core values for the new health care reform can be reflected by experiences of other developed or developing countries. This study put forward policy recommendations in terms of core values of socialism: in the process of the new health care system reform, the objectives of “primary health care for all†should be insisted; universal health coverage oriented by the core values of equal health rights should be achieved; governments’ role and function should be renewed; legal insurance system should be further perfected; a sustainable mechanism should be formed on corruption control and prevention in the field of health; disseminate the values of health care facilities run for people, health care managed according to laws and regulations, health care provision based on moral and love and health care access on rationale procedures; Aiming at the goal of realizing equal health rights for everyone through the new health reform, China should find a new way for health care system reform with Chinese characteristics. From the research, we recommend that: to maintain the “equal health rights†is the value basis and moral bottom-line for health care reform, which is also the basis for health care policy making and implementations. Health care should be considered as public benefit services and a basic human right for all the people. Health care system is not only of economy but also closely relate to politics. Therefore, it is important to implement health care reform under the principles of people centered, maintaining social justice and bringing both government control and market role into play. Chinese government should perfect the healthcare system together with health security as well as facilitate the legalization of health services. As the health care system reform is in development, it is necessary to carry out value analysis on policies and policy measures for the perfection and adjustment of policy making and implementations centered by the core values. Hopefully the research findings will be conducive to the establishment of the core values for health care reform in China. |
